Note: incomplete applications will not be considered for admission

Completed and signed application (including response to three questions located page two)
Check or Money Order for $35.00 application fee payable to Conemaugh School of Nursing
*Three recommendation forms sealed in envelope and signed across the envelope by the endorsers
I have arranged for the following items to be mailed to Conemaugh School of Nursing
Official high school transcripts/GED
Official transcripts of Algebra, Biology and Chemistry courses if not included on high school transcript
Official report of placement test scores (SAT, ATI , ACT, NLN or Kaplan) if not included on high school
transcript
Official transcripts all post secondary education programs (i.e. certificate/vocational programs and colleges)
*Three recommendation forms if not enclosed with application
