PROJECT DESCRIPTION
The County of Amador is seeking sealed bids for the furnishing of
all labor, materials, tax, transportation, services, disposal, equipment,
and appurtenant facilities to provide a landfill closure and gas extraction
system at Buena Vista Landfill near Ione, California. The work shall
consist of the furnishing of all labor, materials, and equipment, including
but not limited to: drilling and installing gas wells, HDPE gas piping and
appurtenances, and earth work including installation of a synthetic cap
cover, erosion control for the construction of the improvements as
detailed in the Bid Form, the General Requirements, and the Technical
Specifications. Said bids shall be in accordance with the Contract
Documents contained herein and with the laws and regulations set forth
by the State of California, building codes and all other applicable
regulations.
INSTRUCTIONS TO BIDDERS
Interested bidders may acquire a bid package at the office of
General Services Administration located at 12200 B Airport Road,
Martell, CA. A bid package (Bid, Contract, Specifications, Drawings and
all Contract Documents) is available for examination at the office of
General Services Administration, telephone (209) 223-6375. A copy of
the bid package and information may be obtained from Amador County
General Services Administration located at 12200 B Airport Road,
Martell, CA during normal business hours (8:00 A.M. to 5:00 P.M.),
Monday through Friday.
The Contract documents including Specifications, Special
Provisions, Proposal and contract forms may be examined at the office of
the Public Works Agency, 500 Argonaut Lane, Jackson, CA. The
contract documents may be purchased in person or by mail from General
Services Administration. The purchase price of each contract document
is twenty-five ($25.00) dollars and is not refundable. Call (209) 2236375. All documents are available only on CD-ROM in PDF Format.

Sealed bids shall be received by the County of Amador Purchasing Office;
by U.S. Mail at 12200 B Airport Road, Jackson, CA. 95642; or delivered
in person, U.P.S., Federal Express, or by any other means to 12200 B
Airport Road, Martell, CA 95654, until 1:30 P.M., July 29, 2004, at
which time, sealed bids will be opened and read publicly in the office of
the Director of General Services Administration, located at the above
Martell address. Bids received late, will be rejected and returned
unopened.
The Board of Supervisors of the County of Amador reserves the
right to accept or reject any or all bids in whole or in part and/or to
waive technicalities and/or any irregularities in any bid received if such
waiver is in the best interest of the County. All bids will become a part of
the official files of the County of Amador without obligation to the County
of Amador.
Bids must be submitted in a sealed opaque envelope. Your return
address and the Bid number
must appear on the outside of the
envelope. Bids must be submitted on the bid form provided in the bid
package. Other bid formats will be rejected.
Contractors must be licensed in accordance with the laws of the
State of California and comply with the Government Code and Public
Contracts Code of the State of California in making this bid. Each
portion of the work shall be performed by a qualified person licensed,
equipped and experienced in the construction of landfill closure and
installation of landfill gas extraction systems. Qualified landfill gas
extraction experience shall mean a person with a minimum of ten
(10) years of experience and a minimum of 10 installations
successful within California in this particular field and/or trade.
Each bidder shall visit the site of the proposed construction, and
shall observe conditions in order to be fully informed as to the materials,
labor and workmanship required and the conditions under which they
shall be furnished and placed. This required pre-bid, on-site inspection
is scheduled for Wednesday, July 21, 2004 at 8:00 A.M. convening at the
Landfill office parking lot.
Should a bidder find any discrepancies, errors, ambiguities or
omissions from the Contract Documents, or be in doubt as to their
meaning, the bidder shall at once notify the County of Amador General
Services Administration Director or the General Services Administration
Deputy Director. Should it be found necessary, the County of Amador
General Services Director shall issue a written Addendum, which will be
sent or delivered to those submitting bids receiving a set of Contract
Documents. The County will not be responsible for oral instructions,

clarifications, or other communications. Questions by those submitting
bids receiving a set of the Contract Documents shall be made at least
seventy-two (72) hours prior to bid opening, not counting Saturdays,
Sundays or Holidays.
Those submitting bids must complete and use the reference form
provided herein listing a minimum of four (4) references, at least one of
which is for the gas extraction well portion. This form should contain
only clients of similar projects and be completed in full.
The project for the work to be done is entitled:

Bids are to be submitted for the entire work.
The bidder shall set forth a bid amount price. The amount set forth
under the "Total Bid" space shall be the total of the bid.
Each bid shall be made on the form provided by the County of
Amador and shall be properly completed with all items filled out, signed
in longhand and sealed.
Notice is hereby given pursuant to the provisions of the Labor
Code of the State of California, that the County of Amador has
ascertained the general prevailing rate of the per diem wages for onsite work to be performed for each craft or type of worker or
mechanic needed to execute the Contract which will be awarded the
successful bidder. The prevailing rates are set forth in the most
current documents entitled "Basic Trade (Sub-Trades) Rates" located
in the County of Amador Public Works Office, and made a part of the
project and the Contract itself by reference.
If this bid is accepted and the undersigned fails to enter into the
Contract and furnish the bonds in the sums required by the State
Contract Act, with the surety satisfactory to the General Services
Administration within ten (10) days, the Department may, at its option,
determine that the bidder has abandoned the Contract, and thereupon
this bid and the acceptance thereof shall be null and void and the
forfeiture of such security accompanying this bid shall operate the same
and shall be the property of the County of Amador.
The successful bidder shall be required to furnish a Performance
Bond in the amount equal to one hundred percent (100%) of the
Contract price, a Payment Bond in the amount equal to one hundred

percent (100%) of the Contract price, and said bonds to be obtained from
an admitted surety under the laws of the State of California and found
satisfactory by the Amador County Board of Supervisors.
Each bid must be accompanied by a certified check, cashiers
check or Bid Bond payable to Amador County in an amount not less
than 10% of the amount bid given as a guarantee that the bidder will
enter into the contract contained herein if awarded to him and declared
forfeited if he/she refuses to enter into the contract contained herein.
If bidder uses any other bond form or bond format other than what
is provided herein, then requested bonding information contained
herein shall appear on all other bonds submitted.
Each bidder shall include all costs of maintaining insurance
complying with the provisions of the Contract.
No person, firm or corporation, shall be allowed to make or file or
be interested in more than one Bid Form for the same work. Alternate
bids for the same work using different methods, and/or equipment,
and/or materials are acceptable.
The successful bidder shall commence work on the Contract on the
date of the Notice to Proceed and complete the project within the
time specified in the Contract Documents.

apply to additional work beyond the scope of work of this project.

expiration date, and type of insurance (claims made or
occurrence).

applicable to County governments under California State laws.
Meet the experience requirements noted in “Instructions to
Bidders” for this bid.

The County of Amador is not responsible for the U.S. Mail or
private couriers in regard to mail being delivered by the specified
time so that a bid can be considered.
Telegraph and Facsimile (FAX) bids will not be considered.
Bids having any erasures or corrections must be initialed by the
bidder in blue ink. Bids shall be signed in blue ink. All amounts
shall be typewritten or completed in blue ink.
BID REQUIREMENTS
This Bid is for the construction of the Work in accordance with the
Contract Documents, including the Contract annexed hereto, the project
Plans, General Requirements, and Specifications.
Bids are required for the entire scope of work. A bid form shall be
submitted based on prevailing rates as set forth in the Bid Forms.
The bidder shall set forth for each item of work, in clearly legible
figures, a unit price or lump sum price and a total in the respective
spaces provided for this purpose.

In case of discrepancy between the unit cost and the item cost set
forth for the item, the unit cost shall prevail, except as provided in (a) or
(b), as follows:
(a)
If the amount set forth as a unit price is unreadable or
otherwise unclear, or is omitted, or is the same as the
amount as the entry in the item total column, then the
amount set forth in the total column for the item shall
prevail and shall be divided by the estimated quantity for the
item and the price thus obtained shall be the unit price.
(b)
If this Bid is accepted and the undersigned bidder
shall fail to contract as aforesaid within one (1) calendar
week after the bidder has received notice from the County of
Amador that the contract has been awarded, or to provide
the required insurance certificate as specified in this
Contract and Special Provisions thereto, the County may, at
its option, determine that the bidder has abandoned the
Contract, and thereupon this Bid and the acceptance thereof
shall be null and void.

part 1, Chapter 4, Section 4100, and following, the subcontractors listed
in this Bid will perform the indicated work or improvement of this

PUBLIC CONTRACT CODE SECTION 10162 QUESTIONNAIRE

In accordance with Title 23, United States Code, Section 112, the Bidder

taken any action in restraint of free competitive bidding in connection with this
Contract.

requirements of the special provisions. Each Bidder thereby certifies that it will
