Upload or provide a web link to your school district’s master calendar for the upcoming academic year. DOCUMENTS SARC 4. Upload most current board resolution of sufficiency of approved standards-aligned instructional materials required in the four core areas, 9-12 science laboratory equipment, 9-12 foreign language, and health for middle and high schools, as appropriate. Board documentation samples available on “Sample Documents” system page. 5. Upload most current Textbook Replacement Procedures (includes replacement procedures for all district instructional materials and science equipment as appropriate) signed by the district liaison or principal. A. Select all district approved instructional materials programs from the system’s list of state board adopted instructional materials in the four core content areas, foreign language, and health, as appropriate. If the program you are looking for is not identified in the list of state board adopted materials, submit a local adoption request (see requirement 7. B.). 7. B. Local Adoption Request (if applicable) Submit a local adoption request along with supporting documentation, by content area, for locally adopted standards-aligned materials in the four core content areas, as well as foreign language, and/or health, as appropriate. An email notification will be issued to confirm the addition of the new material(s) on the appropriate content area page. Continue with requirements 8-10 after verification process is completed. 8. Provide a description of all core materials (i.e. components) for all district approved instructional materials programs by grade level or course. Please do not include non-adopted supplemental component information. (E.g. textbook, Level K consumable, anthologies 1.1-1.5, textbook and practice workbook, big book kit, etc.) Samples of acceptable board documentation can be found on the “Sample Documents” system page. Verify and update school information (e.g. track and grade level information). Provide any comments that you feel the visitation team would find useful. 2. Verify school principal contact information and add alternative contact information. 3. Upload a site-specific school calendar. Please do not provide the district calendar. Teams are looking for dates that may pose a scheduling conflict. 4. Upload bell schedule. 5. Upload school map with room numbers and grade levels identified. DOCUMENTS 6. For elementary schools and self-contained middle schools, upload classroom information by grade level that includes teacher name, room number, student enrollment, and, if applicable, special designations (e.g. Bilingual; SAI; GATE; LH). 7. Select appropriate district approved instructional materials that are in use at the school. MATERIALS 10. For elementary and self-contained middle schools, enter student enrollment (projected enrollment totals are sufficient) and current inventory by grade level for core content areas, foreign language, and health courses, as appropriate. ***Enter total number of classes in the “Section” field. 11.
