20. STATEMENT OF INTEREST: On a separate sheet(s) of paper, please provide a statement of research intent describing
historical fields of study including a discussion of interest and/or experience in media and technology, relevant educational
background, and post-graduate career/educational objectives. Please keep in mind that this is a program that emphasizes history
and new media. Please explain in detail why you are interested in a program with this emphasis.

IN ADDITION to the information on this form, your application must include:

three letters of recommendation sent by people who are familiar with your academic history or professional background and
who can comment on your potential for success in graduate school. At least one letter should be from a current or former
professor. Each of these letters must be accompanied by a completed and signed Letter of Evaluation Form. Be certain to
supply each of your recommenders with the form. Have all letters sent directly to the Department of History.

two sets of official transcripts, one sent to the Office of Admissions and Records and one sent to the Dept. of History.

a copy of the official GRE score report (and TOEFL where appropriate).
