Who should apply?

Interested students in any of the health professions programs (YSN, YSPH, YPA, YSM), who have demonstrated a commitment to the New Haven community and/or who have some experience working in an administrative capacity. Select positions are open to any Yale University student. Applicants must plan to be an enrolled student for the duration of the board term (Jan 2016-mid Feb 2017).

**PLEASE NOTE: Previous experience volunteering at HAVEN Free Clinic is not a necessary prerequisite to these positions. The most important requirement for all applicants for Board positions is significant enthusiasm and commitment. Our patients at HAVEN view us as their health care providers, and we are committed to providing them with the highest possible quality of care. Your colleagues on the

Board will give much of their time and energy to caring for our patients, and they will expect the same from you.

General Board Member Responsibilities

Each HAVEN Director is a member of HAVEN’s Student Leadership Board. General Board Member responsibilities include:

● Attend and actively participate in all Board meetings

● Take an active role in one or more sub-committees, including attending all sub-committee meetings

● Attend monthly department meetings with an Executive Director

● Co-lead at least one Community Advisory Board meeting per year

● Respond to HAVEN-related emails and requests in a professional and timely fashion

● Foster and maintain lasting community partnerships to inform HAVEN’s work

● Monitor and evaluate department-specific and clinic-wide performance

● Develop and execute quarterly quality assurance / quality improvement goals that reflect vision and initiative for your department

● Recruit, select, schedule, record certification of HIPPA, and train volunteers for your department

(if applicable)

● Manage a consistent, effective communication system between your department’s co-directors (if applicable)

● Engage in other Board-wide undertakings, such as strategic planning sessions and fundraisers

● Be supportive, collaborative and flexible

● Take on other responsibilities as they arise

Director Position Descriptions

In addition to serving as a general member of HAVEN’s Student Leadership Board, each Director is responsible for creating a vision and goals for his/her department and overseeing the in-clinic and outside-of-clinic operations to achieve those goals and ensure consistent, quality patient care. Specific responsibilities pertaining to the individual departments are listed below.

Executive Director (2 of 3 positions available)

* Requirements : No specific requirements other than enrollment in a health professions

* Time commitment : program for the duration of the board term (Jan 2014-Feb 2015).

15-20 hours per week. Must be at the Clinic 1-2 Saturdays/month.

* Description : The Executive Directors are responsible for the operation and management of the Clinic. Responsibilities include, but are not limited to:

• Creating HAVEN policies and determining operating procedures with the goal of sustainability and institutional continuity.

• Promoting the development of programs and initiatives in line with the mission and vision of HAVEN and facilitating the development of quarterly goals for each department.

• Serving as a liaison to the Fair Haven Community Health Center, the Yale administration, and the Yale community.

• Meeting monthly with HAVEN’s Medical Directors.

• Meeting monthly with the HAVEN Faculty Advisor at the Yale School of

Medicine.

• Conducting board meetings (every 2 weeks) and Steering Committee meetings (every 3 months).

• Meeting monthly with the Directors of each department (i.e. those listed below) to support departments in their initiatives and ensure follow-up.

• Managing clinic and overseeing patient flow as Session Coordinator 1-2

Saturdays per month at the Clinic.

• Promoting and representing HAVEN at various events (alumni events, volunteer recruitment, press interviews, conferences, etc).

• Promoting board member development and fostering a collaborative working environment on the HAVEN board.

Clinical Advisor (1 of 3 positions available)

* Requirements : Completion of one full year of clinical rotations, including one ambulatory or

Description :

: primary care rotation by September 2014 (can include rising 4th/ 5th year medical students and rising 2nd year specialty nursing or PA students).

Because this position continues into February 2016, students who will graduate in 2015 are not eligible to apply.

10-20 hours per week. Must be at the Clinic every 3rd Saturday. Regular department and committee meetings.

The Clinical Advisors are responsible for bringing clinical insight into the provision of patient care, pursuit of referrals for specialty care, pharmacy management, and clinical data collection. They also oversee some key programs at HAVEN. Specific responsibilities include, but are not limited to:

• Reviewing all lab results and following up with patients appropriately for results; also ensuring all labs ordered are returned

• Leading the training of Senior Clinical Team Members (three sessions per year and as needed); assisting with the training of Junior Clinical Team

Members.

• Helping to facilitate weekly Afternoon Meeting, an educational conference held post-clinic

• Training and supervising Medical Records Specialists who assist in weekly chart review

• Supervising the TB advocates in their work with patients enrolled in the latent

TB infection (LTBI) program

• Supervising the Behavioral Health Advocates in their work with patients enrolled in the Salir Adelante Program

• Helping to make triage decisions for walk-in patients

• Overseeing complex patients that are identified and followed at HAVEN.

• Working as a Senior Clinical Team Member in times of overflow as needed

• Developing and monitoring standards of patient care, as well as measures to increase efficiency and clinic flow

• Conducting an annual chart review with the Research team to ensure that we are providing high quality patient care

• Working with or leading the Clinical Quality Assurance committee to develop interdepartmental initiatives to improve patient care

• Consulting with and reviewing charts with the Referrals team to assist in specialty-care related decision making

• Facilitating transfer of patients in need of closer oversight to FHCHC

• Leading the clinical implementation of the EPIC electronic medical record system in conjunction with the Executive Directors

Information Technology & Communications (1 of 1 positions available)

Communications Director manages the department’s volunteers, and may use their volunteer ’s individual abilities and interests to help complete the department’s responsibilities. These responsibilities include, but are not limited to:

• Support HAVEN’s Epic EMR (electronic medical record), including interfacing with YNHH Epic liaisons for troubleshooting and managing Epic access for board members and volunteers

• Maintenance of the HAVEN website, including regular (monthly or weekly) content updates as well as design and programming of new features

• Developing HAVEN's online presence, including our Facebook page and other online accounts and social media

• Publication and distribution of HAVEN's email newsletter, including design, layout, planning content, and managing board member and volunteer involvement in creating content

• Assisting the Executive Directors in designing and laying out HAVEN's

Annual Report

• Graphic design and production assistance for other clinic publications, including but not limited to: informational brochures, fliers, posters, banners, presentations, and other documents

• Photography (or management of volunteer photographers) at HAVEN events and for HAVEN publications

• IT support including interfacing with Yale IT, managing the HAVEN server at

The ideal applicant will possess:

• Familiarity with web and layout design, basics of graphic design and specs for print products (e.g., file size, image resolution, bleeds, etc.)

• Familiarity with, or willingness to learn, the basics of Epic EMR

Education (2 of 2 positions available)

* Requirements : No specific requirements other than enrollment in one of the health professions programs. Proficiency in Spanish helpful, but not required. Clinical experience will be considered an asset but not a requirement.

5-15 hours per week. Must be at the Clinic every other Saturday (scheduling shared with your co-director) plus 2-8 hours per week outside of clinic for program quality improvement and oversight.

Responsibilities include, but are not limited to:

• Providing one-on-one patient health education on Saturdays, including scheduling and conducting follow-up education, as necessary. The Directors are responsible for managing the following programs: o ANDO (Advancing Nutrition and Diet Outcomes) Program:

Counseling patients regarding nutrition and fitness, hypertension, hypercholesterolemia, weight management, and diabetes.

o Smoking and Alcohol Cessation Programs: Counseling patients

regarding smoking and alcohol use and stress management.

o Clinical Communications Program: Deliver normal and slightly abnormal lab results to educate patients about indications for testing, results, and follow-up.

Faculty Relations (1 of 1 positions available)

* Requirements : No specific requirements other than enrollment in a health professions

* Time commitment : program, though the ideal candidate will have excitement for working closely with faculty volunteers. Post-clinical students are encouraged to apply.

5-10 hours per week. No continuous time required at clinic; however directors who have not previously volunteered at the clinic are required to go to clinic for at least one Saturday. During this time, the Faculty Relations director may

The Faculty Relations Director recruits new attendings to volunteer as preceptors, manages their onboarding process, schedules preceptors for each clinic day, serves as the primary liaison to faculty addressing their needs and concerns, and promotes a rewarding experience for volunteer attendings.

Responsibilities include, but are not limited to:

• Seeking out new physicians/advanced practice nurses/PAs who are interested in becoming preceptors at HAVEN

• Managing the onboarding process for new attendings, which consists of credentialing, privileging, and applying for federal medical malpractice insurance for non-Yale providers

• Ensuring that the Clinic has two HAVEN attendings each week and sending out a weekly reminder email

• Completing and submitting the annual application for federal medical malpractice (FTCA Redeeming application) and the FTCA Supplemental

Deeming application for additional preceptors added during the year

• Keeping all documentation for each HAVEN attending up-to-date and organized

• Organizing an annual faculty appreciation “Thank You” Card-athon

• Organize an annual FHCHC volunteer appreciation day

• Send out HAVEN attendings feedback survey at the end of the year to obtain faculty feedback and report findings to Executive Co-Directors and Faculty

Advisor

Finance & Development (1 of 1 positions available)

* Requirements : No specific requirements other than enrollment in a professional school.

* Time commitment : 5-15 hours per week. No specific time required at clinic; however it is advisable for the finance chair to occasionally serve as a volunteer on Saturdays or to

The Finance & Development director is responsible for securing funding for the operations of the Clinic and overseeing these funds and their distribution.

Responsibilities include, but are not limited to:

• Prompt reporting of the state of HAVEN’s finances, including quarterly financial reports and an annual financial report. Assisting with generation of financial and donor reports for our yearly annual report and Gilead grant application.

• Actively liaising with Yale Finance.

• Creating and managing an annual budget.

• Writing and overseeing grant applications and fundraising with input from

Executive Directors.

• Creating and updating fundraising materials, including an annual fundraising appeal.

• Leading the Sustainability & Development Committee, including spearheading the annual 5K walk/run and GPSCY fundraisers.

• Recruiting and overseeing development volunteers to execute annual fundraisers.

• Facilitating the recruitment of new HAVEN donors and managing relationships with current donors, including tracking donations and sending thank you notes.

1-2 Saturdays of the month not only to ensure familiarity with the Clinic, the patients, and the Fair Haven community but also to serve as a resource and provide feedback to the Interpreting Volunteers. It is useful to be available for filling in as an interpreter on occasional Saturdays when extra interpreters are needed.

Most HAVEN patients are Spanish-speaking immigrants from Latin America with cultural values and life challenges that are different from those of most volunteers and board members. To improve the quality of care provided at the

Clinic, HAVEN is committed to learning more about our patient’s lives, cultural values, and the structural barriers they face to achieving health and wellbeing.

The responsibilities of the Interpretation & Diversity Director include, but are not limited to:

• Recruit and manage interpreters of non-Spanish languages, not limited to, but including: Hindi, Urdu, Arabic, Portuguese and Bengali. (Note: candidate does NOT need to speak these languages)

• Continuing to develop comprehensive training and support program for interpreter volunteers. This includes managing interpreter schedules and communicating feedback.

• Conducting assessments of interpreter applicants’ Spanish proficiency to select a core group of high-quality, dedicated interpreters. Collecting feedback

from clinical teams on interpreter performance to ensure ongoing high-quality communication with patients.

• Coordinating assessments of Spanish ability for all new volunteers for each volunteer recruitment period (3 times annually). Soliciting the help of interpreters or Spanish-speaking Board members to assist in assessments.

• Staying informed of issues affecting the health and wellbeing of Latino immigrants and Fair Haven residents, and engaging the leadership of HAVEN in opportunities for advocacy.

• Participating in board decision-making, new program development, and program evaluation with a focus on diversity and community accountability.

Pharmacy (2 of 2 positions available)

5-10 hours per week. Must be at the Clinic every other Saturday and be available to fulfill Pharmacy responsibilities outside of clinic hours.

The Pharmacy Directors are responsible for ensuring that the Clinic’s patients are able to obtain appropriate medications. Their responsibilities include:

•Coordinating pharmacy donation deliveries with Americares, and other providers

•Overseeing the pharmacy residents program, including scheduling pharmacy attendings and troubleshooting with them in clinic

•Maintaining the pharmacy inventory of donated supplies and medications

•Assisting medical teams with finding affordable drug options for patients

•Coordinating and developing a model for patient education regarding prescribed medications with the assistance of YNHH Pharmacy residents

Laboratory (2 of 2 positions available)

Patient Services (2 of 3 positions available)

* Requirements : Must be fluent in Spanish and enrolled in a health professions program. A

Spanish evaluation will be conducted as part of the application process for those who have not previously done an evaluation as a HAVEN volunteer.

5-15 hours per week. Must be at the Clinic 1-2 times per month. Must be available to answer and return phone calls during the week every 3rd week.

The Patient Services Directors are responsible for:

• Triaging walk-in patients and performing new patient intakes

• Managing clinic flow and the patient waiting room along with the session coordinator.

• Assisting with the patient transfer process from HAVEN to FHCHC

• Collaborating with FHCHC to effectively use scheduling software and maintain organized medical records.

• Scheduling, training, and supervising volunteers who work at the front desk

• Making phone calls during the week to remind patients about their appointments.

• Responding to the appointment phone during the week and scheduling any patients who may call. Clinical phone calls will be triaged with the help of the

Directors, Clinical Advisors, and the on-call Fair Haven Attending clinician.

• Managing the clinic’s appointment schedule on Epic EMR

• Communicating with other departmental directors throughout the week to schedule appointments and coordinate care.

Referrals (3 of 3 positions available)

* Requirements : No specific requirements other than enrollment in a health professions program. Clinical experience will be considered an asset but not a requirement.

* Time commitment :

* Description :

10-15 hours per week. Must be at the Clinic every third Saturday and follow-up on patients during the week.

The Referrals Directors orchestrate specialty care, ranging from surgery to gynecology, for any HAVEN patient in need of services beyond the scope of primary care. Each Referrals Director coordinates a set of cases with specific providers and works closely with a team of Spanish-speaking volunteers, who facilitate patient communication.

Currently, the three Referrals Directors divide case responsibilities by specialty. The Referrals Directors also split up attending Clinic on Saturdays, where they ensure that clinical teams are aware of and able to access the specialty services that the Clinic offers.

In addition to being in Clinic every third Saturday, these positions require a significant amount of time during the week to:

• Oversee the logistics of the various referrals services including referral approval, appointment scheduling, and results follow-up

• Manage volunteers who communicate with patients to confirm specialty appointment dates, provide reminders to ensure attendance, and troubleshoot any patient questions or concerns

• Coordinate with Social Services regarding applications for financial assistance and resolve billing issues when necessary

• Ensure that patients and clinical team members are informed about the details and results of their appointments

• Plan and coordinate specialty screening days in clinic (e.g. vision, physical therapy, and mammograms) to provide access to high demand services

• Recruit specialist providers for new specialties or when a current provider can no longer work with HAVEN, and troubleshoot any issues arising with existing providers

Reproductive Health (2 of 2 positions available)

* Requirements : Requirements include enrollment in a health professions program and prior experience in reproductive health. Candidates with experience and coursework

* Time commitment : in reproductive health topics, including contraception and sexually transmitted infections (STIs), will be prioritized. Proficiency in Spanish is helpful, but not required.

7-20 hours per week (variable), including academic breaks. One Reproductive

Health Co-Director must in clinic each Saturday. In addition to the general board member responsibilities detailed above, at least 5 hours per week during the week will be dedicated to quality improvement/resource development, volunteer oversight, patient triage/follow-up, and working with other HAVEN

* Description : departments (e.g., Referrals, Social Services, Clinical Advisors, Patient

Services). One Reproductive Health Co-Director must be in clinic each

Saturday. On scheduled Saturdays, this is a dedication of 5-6 hours.

The Reproductive Health Directors are responsible for:

• In clinic: o Providing reproductive health education, counseling, and resources to HAVEN patients. Topics include contraception, STIs, test results

(e.g., abnormal pap tests), pregnancy options, and fertility and menopause counseling.

o Consult with medical teams and other HAVEN departments’ volunteers and provide additional counseling and educational resources to their patients as needed.

o Triaging patients to the Reproductive Health Department volunteers and assisting with patient flow and charting.

o Managing Mirena IUD screening and pre-insertion process.

o Dispensing and logging OCPs.

o Charting patient encounters in Epic.

• Patient follow-up as needed during the week. May include working with

Referrals, Patient Services, Clinical Advisors, and Executive Directors.

• Volunteer management, including selection, training and scheduling.

• Reviewing and updating protocols and resources: o Maintaining the Reproductive Health Department electronic resources, including the Google Site and patient tracking system.

o Determining the most effective way to utilize and develop protocols for the reproductive health needs of HAVEN patients in light of the recent transition to Epic. Examples of protocols include those written for Mirena IUD provision, IPV screening and referrals, and pregnancy options counseling.

• Collaboration with HAVEN departments: o Working closely with the Clinical Advisors, Education, Social

Services, and Referrals directors to develop programming and resources to expand and improve reproductive health at HAVEN. o Partner with Social Services to manage patients who experience

Intimate Partner Violence. o Managing pap result and colposcopy follow-up in collaboration with Clinical Advisors and Referrals Directors.

• Designing and conducting the annual chart review, including scheduling and managing volunteers, timely analysis of data, and reporting to the Board and

* Overseeing all data management at HAVEN and leading outcomes ascertainment and metrics for the clinic

• Working with other departments to design and pursue quality improvement initiatives (ex: clinic flow analysis, program evaluations)

• Working with volunteers to develop research project ideas that will advance the mission of HAVEN and be an educational experience for volunteers

• Collaborating with the Student Leadership Board and Medical Directors to design and implement new programs in response to the results of the annual chart review and patient satisfaction survey

• Serving as a point of contact for anyone interested in conducting research at

HAVEN and overseeing all subsequent research proposals per the research policy guidelines

• Maintaining a record of all research projects being conducted at HAVEN and ensuring that all proper documentation and approval has been completed

• Developing a project proposal annually to submit to the Yale School of Public

Health for consideration to include HAVEN as a practicum site for MPH students

• Strong organization and leadership skills

• Overseeing the assessment of impact of volunteering on individual’s proclivity to primary care and community health

The ideal candidate for the Research Director position will possess some or all * Ideal Candidate: of the following qualities and skills. However, all interested students who are committed to the HAVEN mission are encouraged to apply.

• Experience working with or doing research with underserved populations

• Experience designing and implementing studies in health care settings

• Scheduling, training and supervising the Social Services volunteers.

Trainings will include the following topics: insurance eligibility, Free Care application procedures, counseling/interviewing techniques, mental health assessment, and immigration issues.

• Assisting volunteers with documenting patient interactions and formulating an appropriate social service plan for each patient.

• Following active patient cases outside of clinic. This may involve contacting the CT Division of Social Services, local hospitals, and working closely with the Referrals Directors to ensure that the documents required for obtaining specialty care are completed.

• Working with Social Services volunteers to improve the resources available to patients at the Clinic. This may include networking with local organizations or FHCHC staff. Helping connect patients to these resources often requires

some work during the week, depending on how difficult it is to reach various providers/organizations on Saturdays.

• Discussing sensitive issues like intimate partner violence with patients, and working closely with both patients and t he patient’s medical team to connect patients with appropriate community resources.

• Learning about the various federal, state, and community-based programs available for low-income children, families, and adults and the requirements for eligibility.

• Maintenance of the Social Services database, which was developed to keep track of patients seen in the department.

• Overseeing HAVEN’s health insurance screening program and managing the referrals of patients to a community-based organization for enrollment in

Medicaid or a federally qualified health insurance plan.

• Working with the Yale Health Law and Policy Society and New Haven Legal

Assistance Association to maintain and develop the Medical Legal Partnership program for addressing legal concerns of HAVEN patients.

* Requirements : No specific requirements other than enrollment in a health professions program.

* Time commitment : 5-10 hours per week. About 5 hours per week spent emailing volunteers to arrange staffing of the Clinic each Saturday. This increases to 10 or more hours per week during volunteer recruitment and selection, which happens 3

* Description : times per year. It is advisable for the Student Recruitment Director to occasionally serve as a volunteer on Saturdays or to spend time at HAVEN to ensure familiarity with the Clinic.

The responsibilities of the Student Recruitment Director include:

• Addressing prospective volunteer inquiries

• Organizing student volunteer and leadership board applications, info sessions, and recruitment events

• Facilitating volunteer selection process

• Organizing volunteer contact information and schedules

• Scheduling Senior Clinical Team Member, Primary Care Clerkship student, and Junior Clinical Team Member volunteers

• Managing volunteer shift trades, attendance, and concerns

• Sending reminder emails to all volunteers each week

• Lead planning and coordination of a volunteer appreciation event each term, as well as other activities in order to foster a sense of community at HAVEN

(e.g. invitation to volunteers to see a film on primary care)

• Plan and organize Fair Haven Walking tour in spring
