Welcome to the practice
This leaflet tells you about the practice and
the services that we offer. We trust that you
will find it helpful and informative and we
suggest that you keep it in a safe place for
future reference.
We offer a full General Practice Service and
run specialist clinics for children and adults.
We have access for wheelchairs, please ring
and advise when you are attending.
We aim to treat all our patients promptly,
courteously and in complete confidence.
Zero Tolerance - We strongly support the
NHS policy on zero tolerance. Anyone
attending the surgery who abuses the Gp’s,
staff or other patients be it verbally,
physically or in any threatening manner
whatsoever, will risk removal from the
practice list. In extreme cases we may
summon the police to remove offenders
from the practice premises.

Appointments
An appointment system is in operation in our surgery. Please
Note one person per appointment. URGENT cases will always be
accommodated. You may see any doctor in the practice, but it
is better to see one doctor consistently. Children under 16 years
should always be accompanied by an adult.
Please do everything you can to keep appointments and tell us
as soon as possible if you cannot.
Home Visits
If you are too ill to attend the surgery and you require a home
Visit, please telephone the surgery before 10.00 am. The request
will be vetted and patient will be advised of appropriate action the
doctor will take.
Repeat Prescriptions
We require 48 hours notice for a repeat prescription, which must
be made in writing (using the tear off slip from your repeat
prescription).
New Patients
We are happy to register new patients providing they live within
our practice boundary see map provided at the end of this
leaflet. You can register at reception by using a registration form
and supplying us with proof of identity and address.
All new patients will be asked to attend a new patient health
check with the practice nurse. This will be arranged when
registering and failure to attend this appointment will cancel your
registration.

Suggestions & Complaints
We try at all times to provide a service to the highest standard. If you wish
to make a comment or complaint this should be made to the Practice
Manager initially in writing. You will receive a response within 10days.If
you are unhappy at that point you can arrange to come in and

Emergencies
During normal office hours please ring 01782 219075 and a
receptionist will take the details and pass them on to the doctor,
who may wish to speak to you.
Outside office hours, please telephone 0300 123 0814 where
you will get through to the Out of Hours service provided by
Staffordshire Doctors Medical Services, Campbell Road, Stoke.
This should only be used in the case of an Emergency and not
because you are unable to attend in normal surgery hours.
Please note that all calls will be recorded.

1. You will be treated as an individual, with courtesy and
respect, regardless of your ethnicity, religion or cultural
beliefs.
2. We operate an appointment system. We aim to provide
same day appointments if possible. Urgent problems are
dealt with as appropriate.
3. We aim to see you within 30 minutes of your appointment
time. If there is a delay and explanation will be given
4. We endeavor to offer an appointment with the practice nurse
within 5 working days.
5. Repeat prescriptions will be ready for collection in 48 hours.
Urgent request forms will be dealt with the same day.
Telephone requests will not be taken.
6. We aim to deal with telephone calls as efficiently as possible,
and ensure that there are sufficient staff to do this. You can
request to speak to the doctor by telephone.
7. If you have undergone tests or x-rays ordered by the practice
we will inform you of the results at your next appointment. If
no further appointments needs to be arranged, we will advise
you when and how to obtain the results.
8. You have the right to see your health records, subject to
limitations in the law. These will be kept confidential at all
times.

1. Please treat the doctor and the staff with the same courtesy
and respect with which they treat you.
2. The practice has a no- tolerance policy for violence or abuse
to the practice staff or premises, and patients will
automatically be removed if this occurs.
3. Please do not use emergency appointments for routine
matters or the renewal of repeat prescriptions or medical
certificates.
4. A normal appointment is for 1 person only. Please make
separate appointments for each person wishing to see the
doctor.
5. Please arrive on time for your appointment. If you are unable
to attend or are delayed, please inform the surgery.
6. Please order your medication 48 hours before you need it!
Simply return the prescription counterfoil to the surgery
ticked with the items you require.
7. Please keep all call brief to free the lines for someone else
and avoid calling during the peak morning time for nonurgent matters.
8. Please let us know if you change your name, address or Tel
Number as soon as possible.
9. Please let our staff know when you are pleased with our
service, not just when things go wrong
10. Remember, you are responsible for your own health and the
health of your children. We will give you our professional
health and advice. Please act upon it.
Confidentiality

ALL patient information is handled confidentially
according to NHS guidelines. You have the right to
access any personal information held under the DATA
Protection Act but a charge may be payable. We will
not disclose any details with other agencies unless we
have your written consent.
