DOG/CAT LICENSES ARE DUE FOR RENEWAL IN JANUARY
All dogs and cats over the age of 6 months must be licensed every year in
January before January 30th to avoid incurring a late fee. A $25.00 late fee will be
assessed for all licenses acquired after January 30th.
To obtain a 2016 license, a dog/cat’s rabies shot MUST have an expiration
date of 10/31/16 or later. Any dog/cat with a shot expiring before that date must
be re-boostered before licensing. This is a State law.
You may obtain a license at Borough Hall, 11 Asbury Avenue, Monday through
Friday from 8:30 AM to 4:00 PM, or by completing the following application, one
for each license needed, and returning the application form(s), together with your
dog/cat’s rabies vaccination certificate showing the expiration date, with a check
made payable to the “BOROUGH OF FARMINGDALE” and a self-addressed,
stamped envelope to:

Dog/Cat licenses expire January 30th of each year. Please mail this application to the
Borough of Farmingdale, 11 Asbury Avenue, Farmingdale, NJ 07727 before that date,
together with proof of rabies, a SASE and a check for the correct amount for each
dog/cat being licensed. Please complete a separate application for each dog/cat. If you
