WELCOME TO DR SAINI’S PRACTICE
Dr Mohan S. Saini trained at the armed forces Medical College at
Poona, India and entered the Practice in 1981. His interests lie in
diseases of the eyes (ocular pathology).
Dr Jessy Saini trained at the University of Birmingham Medical School
and has qualified since 2002. Her interests lie in minor surgery, family
planning and women’s health.
The doctors provide an excellent quality of general medical service to all
their patients.
WHAT DOES YOUR PHARMACY OFFER?
Before making any appointment to see your GP, Pharmacy First is a
new scheme, which can offer a variety of treatments for your minor

PRACTICE CHARTER
Below is a statement of your rights and responsibilities as a patient at
this surgery. Your rights are:



To be registered with a family doctor
To change your doctor easily and quickly
To be offered a health check:








When joining a doctor’s list for the first time and
aged over 5 yrs old.
If you are between 16-74 yrs and you have not seen
your doctor in the previous 3 years
A yearly visit if you are over 75 yrs old or over and
unable to attend the surgery
To receive emergency care at any time through a
family doctor
To have all the necessary and appropriate medicines
prescribed.
Be referred to a consultant acceptable to you when
your family doctor thinks it is necessary and to be
referred for a second opinion if you and your family
doctor agree this is desirable.
To have access to your health records, subject to
limitations in law.
To know that those working for the NHS are under a
legal duty to keep the contents of your health
records confidential.

To choose whether or not to take part in medical
research or medical student training.
To be given detailed information about local family
doctor services through your Family Health Services
Authority’ local directory.
To receive a full and prompt reply to any complaint
you make about NHS services.
To be treated with respect and dignity.
Not to be discriminated on grounds of face, gender,
disability, age, sexual orientation or medical
conditions.
REGISTER WITH THE PRACTICE
If you live within the practice area we cover, you will be accepted as a
patient by completing your medical card and ethnicity form. If you are
unable to produce a medical care, we will ask you to complete a GMS1
form. Patients from abroad will need to produce their passport/ID card
and proof of address before registration can be completed. All new
patients over 5 years of age are required to have a general health check
on the day of registration.
This practice complies with the Data Protection Act and Access to
Medical Records Legislation. Consent is required for information to be
given to a third party. Identifiable information will be shared with
others to provide medical treatment e.g. district nurse and hospital
services. We also have a duty to others e.g. child protection cases.
You will be treated with respect and as a partner. Being a partner
means you have responsibilities too. We will ensure that our patients
have 24 hours access to medical advice by a suitable qualified medical
professional.

Patients, who are abusive/violent towards staff, will be removed from
the practice, whilst on premises. This will be done by the practice
completing a PVP1 form for instant removal. You will then be sent
information by the tPCT on how to register with a doctor.
PRACTICE STAFF
Practice Manager - Mrs Lorna Dooley-Lynch
 Carries out administrative problems including complaints and
to supervise the running of the practice.
Practice Nurse – Mrs Ruth Alam (RGN)
 A qualified nurse who deals with your health concerns and
health conditions. She also runs the Chronic Disease
Management clinics alongside the doctors.
Health Care Workers – Mrs Shanaz Begum (Senior Receptionist) &
Mrs Mandeep Kaur (Receptionist)
 Work under the supervision of Dr J/M Saini. Trained to do
general health checks, including blood pressure, urine check
and blood taking.
Senior Receptionist – Mrs Ravinder Saini
Receptionists – Miss Saima Mirza & Mrs Gulshanjot Kaur
 Are all able to help you with general enquiries, booking
appointments and taking your repeat prescription requests.

HOW TO SEE THE DOCTOR
You can book an appointment to see the doctor by telephoning or
coming into the surgery during the surgery opening hours:
9:00am – 12:00pm
4:00pm – 6:00pm
3:00pm – 5:00pm

Thurs
You will be offered the earliest available appointment. The doctor also
does telephone triage. The doctor will return your call and decide if you
need to be seen on the same day or offered an appointment on
another day. If you are too ill to make an appointment, you will be
advised to attend the surgery at 11:30am (the end of surgery) and will
need to wait patiently until you are called by the doctor.
HOME VISITS
If you are too ill to attend the surgery, and you would like a home visit
by the doctor, please contact the surgery before 11:00am or as soon as
possible, so that the doctor can plan the visit for the day.
WHILST ON THE TELEPHONE
Remember that the receptionist is trying to provide the best service the
practice can offer, so please be as helpful as you can. Please inform the
staff if you wish to speak to the doctor and the doctor will return your

call as soon as possible. This can only be done between 9:00am 10:00am (Mon – Fri).
WHEN IS THE SURGERY CLOSED
A doctor is on duty at all times to deal with emergencies. If you should
need this service please contact the surgery and you will be put straight
through to the out of hours emergency service Primecare (Direct
Telephone No. 0845 603 176). Please remember that night visits are
very tiring to the doctor who may have been on duty the day before or
will be on duty the next day. Therefore, night visits should be
requested only when absolutely essential.
REPEAT PRESCRIPTIONS
Requests for repeat prescriptions will not be accepted over the
telephone. This will only be done for house bound patients. Please
retain your computerised repeat prescription slip and when you have a
week supply of medication left, tick the box on your computerised
repeat prescription slip of the medication required. Your prescription
will be ready for collection in 48 hours. If you have lost your
computerised repeat prescription slip, only then you may bring in
medication containers or make a note of your medication requirements
and please include your full name and address. We do have a repeat
prescription box situated in the main corridor, which is checked twice
daily by reception staff. This can be used at any time including when
the surgery is open or closed. This will save you waiting. If you have
arranged for your prescription to be collected by the
chemist/representative, please inform them of the surgery opening
times.

CHANGE OF ADDRESS & CONTACT NUMBER
Please inform the practice of change of address and contact
number so that we can update our data. A list of doctors can be
obtained from the Primary Care Shared Services Agency Tel No.
0845 111 1200.
TEST RESULTS
Please contact the surgery after 7 days for your test results. We
suggest you contact the surgery between 11:30 am – 12:00 pm
(noon).
HOSPITAL REFERRALS
If the doctor decided that a hospital referral needs to be done for
further management of your condition, you will be asked to
choose and book your appointment. Instructions will be given to
you so that this can be done.
If you frequently miss your hospital appointment, you must have a
good enough reason for the doctor to refer you again, as this is
wasting NHS resources.
PHOTOCOPY OF CORRESPONDENCE ABOUT YOUR CARE
If you require any correspondence copy from your medical
records, please inform the receptionist. There will be a charge
depending on the amount of copies required.
If you are unable to keep your surgery appointment, please inform
us as soon as possible so that the appointment can be offered to
someone who needs it. If you miss two surgery appointments, you

will not be able to make an appointment with the practice. You
will be advised to come to the surgery and wait to see the doctor.
COMPLAINT/COMMENTS
We welcome your comments in the comments box situated in the
main corridor. All complaints should be addressed to the Practice
Manager (Mrs Lorna Dooley-Lynch).
If you feel that we have not dealt with your complaint
appropriately, you may contact PALS on 0121 255 0707, to help
you resolve any problems before they become formal.
