National Diabetes Audit (NDA) 2014-15
Secondary Care (hospitals) data submission
The NDA 2014-15 data collection window is open until 11 September 2015. You can submit
your data in one of the following ways. If you want to submit Insulin Pump data to the Core
NDA data please liaise with colleagues in your diabetes team, who will be submitting data
for all diabetes patients who are registered with your hospital.
1. Data extraction from a Diabetes Management System
Data required for the audit can be extracted from Diamond and Diabeta3 diabetes
management systems.
For out-patient units using Diamond please contact Hicom for assistance on 01483
794945 or at support@hicom.co.uk
For out-patient units using Diabeta3 please contact David Lewis for assistance at
david.lewis@hisvector.com.

2. Data extraction from a hospital EPR system
The Secondary Care CSV Specification can be used to extract NDA required data from a
hospital EPR system. This is available on the NDA website here

3. Data extraction from a hospital PAS (Patient Administration System)
If you work in a service that still uses paper records, you can still participate by sending a
list of the NHS numbers of the patients that have attended your clinics. It should be
straightforward to extract these from your hospital PAS.
A CSV file holding only NHS numbers for these patients with an identifier (Y/N) for those
patients using insulin pumps (if relevant) can be sent via email from an nhs.net account
to diabetes@nhs.net
Your reports will then be based on the care processes and lab results recorded
by the GPs of these patients.
4. Manual data extraction
The Secondary Care CSV Template can be used to manually collect the data required by
the NDA. This is available on the NDA website
