of all field experiences and professional activities that were
completed during the semesters in the licensure program at Liberty
University.

Click on “To order online” and check “Hold for degree”. All candidates are required to request one paper format transcript to be
sent to the Teacher Licensure Office. (There is a $10 fee to have this sent through intercampus mail.)
Submit your resume to www.liberty.edu/lunetwork so school systems looking for educators can view your resume on LU Network.
Any schools seeking educators will be referred to LU Network so our candidates will be able to view employment opportunities.
Graduation Application must be submitted online through ASIST. (From the student tab in ASIST, click on "Student Records" and then
"Apply To Graduate".) The application must be completed by March 15th in order to guarantee the student's name will appear in the May
Commencement Program. The graduation application must be completed before your degree will be conferred.

Check with Student Accounts Office: Accounts must be paid in full before any transcripts are mailed.
Other state licensure forms can be downloaded from: http://education.uky.edu/AcadServ/content/50-states-certification-requirements from the
state’s website. There is usually a letter (Ohio) or a verification form (NC, SC, TN, WV, etc.) that needs to be completed for the dean’s signature. Please
send me a written request listing your current demographic information (full name, address, LU ID#) and the state you are going to apply for a license. As
soon as I receive your degree-conferred LU transcript, I will complete the form for the dean’s signature and mail it back to you to submit with your
application.
The documents listed below on Part 2 are required ONLY when applying for the Virginia license.
Please DO NOT mail in your own VA application to the VDOE. The application MUST be mailed from “the college or university where you
completed a Virginia approved program”. Please mail all documents or drop off at the Teacher Licensure Office (DH 1165E). Please
DO NOT send these forms as e-mail attachments or faxes.
Print and complete the first form in this application packet (pg. 7-8) with your signature and date completed on both pages. Down

Report On Experience form - (Last page of VDOE application in link above)
A report verifying experience must be completed by the appropriate nonpublic school or public school division official verifying full-time
employment, under contract in the public schools or accredited nonpublic schools. The endorsements that require this form for licensure are
noted below:
Administration/Supervision: 3 years as a full time “licensed” classroom teacher – Submit copy of teaching license.
School Counselor: 2 years full time teaching or counseling experience; however, without any previous experience in an accredited school
you will receive a “letter of eligibility” from the VDOE in order to be hired as a school counselor under a VA provisional license.
Reading Specialist: 3 years teaching experience with reading responsibilities – Submit copy of teaching license.
Math Specialist: 3 years teaching experience with math responsibilities – Submit copy of teaching license.
Early Childhood or Gifted Specialist: No experience required, no form needed. – Submit copy of teaching license.
Submit official transcripts from all other institutions where you completed undergrad coursework. (If you have a
Virginia teaching license, this may be submitted in lieu of these transcripts since the VDOE would already have these on
file.) Please request these official transcripts to be mailed to you so you can include the “sealed” transcript envelopes with your other Gate 4
paperwork that you mail to the Teacher Licensure Office at Liberty University, Box #710098, 1971 University Blvd., Lynchburg, VA
24515.

First Aid/CPR/AED training documentation is available through the Red Cross or American Health Academy https://cpraedcourse.com/
You will need to complete the CPR/AED course ($19.95) and the First Aid Course ($19.95) (Training for First Aid/CPR/AED is required for
ALL VIRGINIA licensure applications submitted after July 1, 2013).
