RESUME: A detailed resume indicating: a) name and address of employers, b) dates of employment, c) detailed job description/duties,
d) post secondary education including dates completed and degrees received.
EDUCATIONAL DOCUMENTS: Copies of all university-level and post graduate educational documents (diplomas, transcripts, course
lists, marks, etc.). Both the diploma and academic transcripts are required. Any foreign educational credentials must be evaluated by an
official evaluation agency/company. If the foreign educational credentials were already officially evaluated, a copy of the evaluation report
must be submitted.
COURT AND PRISON RECORDS (if applicable): A certified copy of each court record or any prison record must be obtained for any
criminal conviction(s).
I-94 (ARRIVAL AND DEPARTURE CARD): If in the U.S., current copies of Forms I-94 (front and back).
PASSPORTS: Complete copy of current passports (including blank pages) as well as any expired passports containing U.S. visa(s).
FORMS I-797 Notice of Actions or I-129: If current or prior TN, H or L, provide copies of all Forms I-797 (approval notices). If entered
on blanket L, provide copy of stamped I-129.
MEDICAL LICENSE/CERTIFICATES: Please provide any Medical Certificates, the ECFMG Certificate, Florida Medical License and/or other
certificates.
PERMANENT RESIDENT: Please provide any documentation such as PERM application, I-140 receipt/approval notice, I-485 receipt notice, I-131
receipt/approval notice and I-765 receipt/approval notice for permanent resident Additional time in H-1B based on Permanent Resident: Need copies of

If changing to TN from TD: If spouse is TN and changing to TN. Please provide copies of spouse’s last three months pay stubs, marriage
certificate translation and English, passport, I-94, I-797 Notice of Actions. There may be additional information upon request.
F status: Previously held F-1 status, provide copies of all Forms I-20.
Employment Authorization Document(s) – EAD cards. J, F or permanent resident

document issued in a foreign language must be accompanied by a complete English translation that is signed by
a competent translator. These translations must be accompanied by documentation that addresses the
competency of the translator who provided the translation from the applicable language to English.
*Transcripts: A transcript must also be included for all US degrees.
*Evaluation - Foreign degree(s) must be evaluated by a US agency. Diploma of highest degree earned or the
degree that qualifies the individual for the applicable position. Any document issued in a foreign language must

FRAGOMEN WILL CHARGE A LEGAL FEE FOR EXPEDITED/RUSH FEE CASE PROCESSING (WITHIN 4 WEEKS FROM ALL
OF THE FOLLOWING BEING MET/COMPLETED: (1) BOTH EMPLOYER AND EMPLOYEE QUESTIONNAIRES RECEIVED, (2)
ALL REQUIRED SUPPORTING DOCUMENTS FROM BOTH EMPLOYER AND EMPLOYEE RECEIVED AND THIS IS NOT A FEE
TO USCIS OR UF. THIS IS A FEE TO FRAGOMEN.
To request a University issued check(s) for processing fees the departments/centers must submit the Alternate
Check Delivery Form and Invoice for USCIS Processing Fees to the UF Disbursements office along with other
paperwork. Please make sure the checks are returned to you and you forward to Pynie
Pynie will submit the information/documentation in the Immigration Home Page (IHP) once she has received
the first part of the process. The first part has to be completed and submitted to Pynie properly or it will not be
submitted in the IHP system. The department does not have to complete any other forms at that time.
Pynie will send an email to the department and foreign national that the employer’s part has been submitted.
Fragomen will send an email to the foreign national to complete their part of the process. It is imperative that
the foreign national provides the correct email address and that they monitor their emails.
Fragomen will complete the necessary forms for processing. The department does not complete the I-129 form
and etc. Fragomen, Immigration Compliance and COM/Adm Office will work together to process and file the
application to USCIS. COM/Adm will contact the departments if any information/documentation is required.
All initiations for H-1B, E-3, O-1 and TN must go through Fragomen’s IHP system. For non-rush cases,
ALL of the following must be met/completed: (1) Both Employer and Employee Questionnaires received, (2)
all required supporting documents from both Employer and Employee received. After Fragomen receives the
signed forms (from Immigration Compliance), any additional supporting documentation, and USCIS filing fee
checks, their normal processing time to send the petition to USCIS is 2 days for expedited/rush cases and 3-4

days for non-rush cases.
Fragomen automatically indicates three years on the TN application.
The foreign national completes the dependent information in the IHP once they receive the email from
Fragomen. We/COM do not complete the dependents form. Please note that if the TD application for
dependents is filed separately from the TN application than the legal fee to Fragomen will be $850.
