Living Faith: An Anglican Community invites you to be a part of our 9th Annual Golf Tournament, a community-based
partnership that works on behalf of our neighbors around the city. The tournament will be on Saturday, November 14th,
2015 at the Rolling Hills Golf Course in Tempe, from 7:45 am to 2:00 pm.
We are excited about this golf tournament, because it leverages the efforts of local businesses, community agencies,
churches, and individuals who serve this cause in these ways (Details for these involvement options are on the next page):
Form a 4-some to play ($70.00 per golfer prior to Oct 31, $85.00 per golfer after Nov 1, 2015) –Fee includes green
fee, cart, range balls, prizes and awards lunch following tournament play.
Sign up for the Tournament lunch only ($20.00), for non-players - includes 10 free raffle tickets
Sponsor a hole ($200.00) - sponsor’s name listed in the line-up the day of play and 8”x10” sign placed at one of the
tee boxes
Become a Corporate sponsor ($400.00)- hole sponsor benefits on a 17” x 22” sign with your logo placed on the
driving range outside the Pro Shop
Donate silent auction and raffle items (each donating business will be listed with the item)
We are pleased to announce a Hole-in-One contest - prize of $20,000 for any golfer making a Hole-in-One on a select hole.
The sign-up form is included. Proceeds will extend and expand our community work, including:
The community of Guadalupe, working together using asset-based community development to provide educational
and youth development support for ages 2 years to 20 years.
Students at Arizona State University, enabling International students to receive culture training, language practice,
and spiritual encouragement, and graduate students facing life’s major questions to be mentored and encouraged
spiritually.
Prison Ministry, personal mentoring for inmates, and communities formed for spiritual encouragement / learning.
Adopt-a-School, partnering with Kyrene del Norte to address the felt needs of the school, students and families.
Feed My Starving Children, children and adults packing meals specifically formulated for malnourished children
which are shipped to nearly 70 countries around the world, including the United States.
To our donors and sponsors: You will be publicly recognized during the event. We want those who participate to know that
they are part of a generous community that cares for people in need.

Golfers
Tournament Format: Four player scramble
Cost: $85 each golfer; $70 Early Bird Entry (by October 31st)
You may register ahead without knowing all golfer details, but we must have complete names and information on
all four golfers by 5 p.m. Wednesday, November 12.
Please arrive by 7:30 a.m.; Shotgun Start is at 7:45 a.m. sharp!
Dress Code: Shirt with sleeves
Hole-In-One Cash Prize of $20,000!
Lunch Only (for Non-Golfers)
The lunch buffet at the clubhouse sports grill will open by noon
Cost is $20. This also includes 10 free raffle tickets (a $10 value) for valuable prizes
