 I am able to fully attend all five days of the Arts Integration Learning Lab and associated
activities, from 9am to 3pm on Monday, March 14, 2016 through Friday, March 18, 2016.
 I understand that part of the Arts Integration Learning Lab’s success requires my full effort and
contribution in activities and discussions, as well as my willingness to share.
 I consent that portions of these workshops may be photographed or filmed.
 I am a resident of the Greater Vancouver area (or commutable distance).
 I am a professional artist (see definition below).

A professional artist is defined as someone who has specialized training in the arts (not necessarily in
academic institutions); is recognized as a professional by his or her peers (artists working in the same
artistic tradition); is committed to devoting more time to his or her artistic activity, if financially possible;

Arts Integration is an approach to teaching in which students construct and demonstrate understanding
through an art form and engage in a creative process which connects an art form and another subject
area and meets evolving objectives in both. (Source: The Kennedy Centre)
