Purpose:
Facilitate, develop, monitor, measure, and report on educational programs to compliment the On-Site
Disease Management and Episodic Care Clinics maintained and coordinated by Healthstat, Inc. In
addition, the Health Coach will present information about how to improve health, prevent disease, and
modify behaviors to individual patients as well as client groups. They will also be responsible for creating
and writing educational materials, conducting meetings, and planning informative, interesting, and
effective programs. These duties are carried out for a specific client at their various locations.
Essential Functions:














Develop and promote Physical, Mental, and Emotional Health Education Programs as needed
Develop and promote programs emphasizing wellness and prevention and that emphasize the
importance of healthy lifestyles. Some of these program topics will include tobacco cessation,
diabetes management, hypertension management, COPD management, Cardiac Management,
Exercise and Physical Fitness Development, Weight Loss Management and other topics. The
programs should include step-by-step implementation plans, program details, resources needed,
goals for success, and all tools needed to successfully complete the program
Develop a plan to educate the on-site providers about Disease Management and Wellness modules,
including protocol for BMI measurements and tracking
Assist in planning client wellness fairs including speakers, health-related demonstrations, displays,
and video materials provided by local agencies, insurance carriers, local/regional agencies, hospitals,
and other resources
Measure, monitor, and report on efficacy of all Health Related Programs/Modules of Healthstat
Review client HRA information, as requested, and consult on wellness information, compliance
program design, and effectiveness of wellness programs
Draft executive summary on HRA summary report
Will work with individual patients after the completion of HRA’s to provide coaching on HRA results
with the goal of helping the individual change their lifestyle and improve risks
Present and discuss Health and Wellness programs to assigned client and to discuss Health Related
topics in an effort to add value to and obtain revenue from the prospective client
Consult on wellness and disease management topics at the request of other benefit consultants,
brokers and others in the wellness industry
Prepare and distribute a list of community health agencies’, hotlines for drug/alcohol abuse, abuse,
and crisis intervention to be available to and used by the on-site clinic providers in their contacts
with patients at the assigned client site
Develop and maintain a database to stay current on new, innovative techniques to promote
wellness and chronic disease prevention and management
Determine availability of governmental grants to help fund the R&D of these programs
Perform other duties as assigned
Essential Physical Demands:








Ability to sit at desk in office location each normal work day
Ability to use personal computer
Ability to communicate clearly and work effectively in person and by telephone
Ability to travel commercially by common carrier
Ability to work well in a fast paced environment, while managing multiple priorities and meeting
deadlines
Ability to drive to and from client and vendor sites if needed
Ability to lift and pull 25lbs
Ability to travel 20% of the time
Essential Environmental Demands:


Ability to work in climate-controlled office environment
Ability to conduct client meetings, on occasion, in a variety of environments
Qualifications:









A bachelor’s degree from an accredited institution in health education or another medical, wellness,
fitness or health-related subject
Master’s degree in health education or related subject a plus
Health or wellness certifications are a plus
At least 2 years’ experience in developing, monitoring, measuring, communicating, and reporting on
health related topics and programs are a plus
Demonstrated competence in multi-tasking
Experience leading and developing presentations. High energy personal style and aptitude for
process-oriented thinking
Track record of consistent, stable employment along with the ability to work independently and as a
part of a team
Demonstrated passion for health and wellness
Demonstrated proficiency in use of Microsoft Office products
Relationships:


High visibility and communication with clients, prospects, vendors, and carriers
Internal contact with producers, client managers, clinic personnel and support staff
