Sent Developmental Alert (KHS-127) and copy of Developmental Screen to
Disability Coordinator.
Permission for Referral ……….(KHS-146)
Parent’s Rights (KHS-148)
Is this conference documented on Family Contact Record (KHS-901),
Disability Tracking Sheet (KHS-157) and Galileo
Sent “Notice to LEA – Children on a current IEP” (KHS-158)
This date is to be recorded on the Disability Tracking Sheet and Family Contact
Sheet.
When IEP Review is Due
Send or take, “Parent Invitation to IEP/Pre-Referral Meeting” (KHS-149)
Send or take, “Parents are Very Important in the IEP Process”.
Take the Recommendations of the Multidisciplinary Team (KHS-150)
(All team members should sign it. The professional in charge of the meeting should
write the recommendations on the back.
Send a copy of the IEP, RMDT, (KHS-150) and Notice to Parents (SDE form 8) to
Disability Coordinator’s Office.
Transition Process
Notify LEA of children ready for Transition (KH -154)
Send or take ”Parent’s Invitation to Trans. Planning Meeting” (KHS -155)
Parent should sign this and return it. Keep it in the child’s file.
Take to the Transition Meeting
Current Developmental Assessment
Transition Planning Meeting (KHS -156)
Put a copy of KHS -156 and Notice to Parents if completed, in child’s file and send
a copy to Disability Coordinator.
Child’s file should contain
