Initial 504 Process
1. Site 504 Coordinator receives a Section 504 Referral.
2. Site 504 Coordinator collects data to discuss at the 504 evaluation meeting. Examples
include: student grades, discipline records, AIMS results, attendance, work samples
3. Site 504 Coordinator sends Parent Invitation for 504 Meeting and Parent’s Rights and
Safeguards Under Section 504. Invite people knowledgeable about the student.
4. At the meeting, obtain Parent Consent for 504 Evaluation and offer Parent’s Rights and
Safeguards Under Section 504.
5. Members of the 504 Team review data and complete the Section 504 Eligibility
Determination Report. If the student is found eligible, the team develops a Section 504
Student Accommodation Plan. To mark the student accommodation plan with a red check
mark as most recent and lock all documents.
6. The Site 504 Coordinator is responsible for ensuring that applicable staff has access to
the 504 plan.
7. Provide parents a copy of all documents.
504 Tab and Flags in Infinite Campus.
When completing the 504 process in e-IEP PRO, in order have the student’s 504 plan show up in the 504
Tab and the alert flag show up in Infinite Campus, make sure the following is completed in e-IEP PRO.
The 504 checkbox must be checked on the Student Profile > Profile Info
The 504 Check box is under General Information half way down the page.
The 504 Student Accommodation Plan must be locked.
To inactivate the 504, simply remove the 504 check box from the Student Profile > Profile Info page. It is
not necessary to delete or unlock the 504.
Annual Review of the 504 Accommodation Plan
(Review 504 Plans at least annually, or if new information is received that indicates a need.)
1. Site 504 Coordinator sends Parent Invitation and Parent’s Rights and Safeguards Under
Section 504 for 504 Meeting. Invite people responsible for implementation of the plan
to discuss how they have implemented the 504 accommodations, and any concerns
they may have.
2. At the meeting, the team reviews the plan and determines if any changes need to be
made.
3. Site 504 Coordinator: “copy up” the 504 Accommodation Plan and make the necessary
changes:
a. IEP-PRO student page: Select the folder icon to the left of the current 504
Accommodation Plan. A copy of the plan will appear.
b. Open and make any necessary changes.
4. Provide a copy of the new Accommodation Plan to the parent/s (and student). The Site
504 Coordinator is responsible for ensuring that applicable staff have access to the plan.
3 year Re-evaluation of 504 Eligibility
(Complete an evaluation at least every 3 years, or when new information is received that
indicates a need.)
1. Site 504 Coordinator sends Parent Invitation and Parent’s Rights and Safeguards Under
Section 504 for 504 Meeting.
a. Invite people that have been responsible for implementing the plan. They need
to be prepared to discuss how they have implemented the 504 accommodations
and any concerns they may have.
b. Collect data to present. Examples include: grades, discipline records, AIMS
results, attendance, work samples
2. Obtain Parent Consent for 504 Evaluation and offer Parent’s Rights and Safeguards
Under Section 504.
3. 504 team members review data and complete the Section 504 Eligibility Determination
Report.
4. If the student remains eligible, update the Section 504 Student Accommodation Plan.
The Site 504 Coordinator is responsible for ensuring that applicable staff have access to
the 504 plan.
5. Provide parents a copy of all documents.
Process for Transfer 504 Plans
(A student comes to CUSD with a 504 Plan.)
1. 504 team reviews the 504 Plan and determines if all accommodations listed are
appropriate. If not, the team determines another way the need can be met. (i.e: use of
a trampoline in the corner of the classroom for sensory breaks. If the team determines
that inappropriate for the classroom, then the team will need to devise another way to
meet the student’s needs in this area.)
2. Previous 504 accommodations from an existing plan should be added to IEP-PRO to
ensure access to all staff. Additionally, scan the existing 504 Plan into IEP-PRO
supporting documents.
3. The Site 504 Coordinator is responsible for ensuring that applicable staff have access to
the 504 plan.
4. Provide parents with a copy of the CUSD Section 504 Student Accommodation Plan.
