Interim Guidance
HOW TO COMPLETE THE NOTIFICATION OF DRAIN MAINTENANCE OR REPAIR FORM
Section 1: Contact Information, Location and Dates for Proposed Works


Complete the contact information, location of the work and start/completion dates:
Please attach a drain classification map and highlight the area for the proposed work
Section 2: Species at Risk Information




Identify whether the drain is covered by an Endangered Species Act Exemption
Identify whether there are Species at Risk under the Species at Risk Act
Indicate which species at risk are present (e.g., turtles, fish, etc.)
For Federal Species at Risk (fishes and mussels) – attach a species at risk map to show the
location of the work in relation to Species at Risk http://www.conservation-ontario.on.ca/whatwe-do/watershed-stewardship/aquatic-species-at-risk
Section 3: Drain Classification


Indicate the drain class in the work zone and impact zone (work zone is where the work is being
completed; the impact zone is 1 km downstream of the bottom end of the Work Zone)
For DFO’s Purposes only: If the Drain is a Type F (Classed as a Type F on the Drain Maps as
having low flow and no Species at Risk in the Impact Zone - DFO does not need a copy of the
Notification Form). Note: All other Agencies do require a copy of the form.

Section 6 (a – c): Sediment and Erosion Control Measures to be Used (Avoidance, Mitigation and
Offsetting Measures (Fisheries Act))
These sections are used by All Agencies. For DFO’s purposes the sections have been renamed to meet
Section 6 of the Fisheries Act where the applicant needs to demonstrate they have considered
avoidance measures and included mitigation and offsetting measures for each Class Authorization.

This section does not need to be completed for Class F
Class D, E and SAR Drains require a site specific review by DFO and a biologist will work
with you to determine appropriate mitigation and offsetting.
Select the appropriate offsetting options that will be successful
The number of offsetting measures and quantity should offset the scale and extent of
the drain cleanout
