USER AGREEMENT FOR GRAPHING CALCULATORS 2015-2016
Henry Clay High School is pleased to offer students the at-home use of graphing calculators. Students under the age
of 18 must obtain parental permission and must sign and return this permission form to the LIBRARY. Students 18
and over may sign their own forms.
Students are responsible for using this equipment at home with care, just as they are in the classroom. The calculator
is provided for students to assist them in their academic work. Access to the equipment is given to students who
agree to use it in a considerate and responsible manner. Parent permission is required. This use is a privilege, not a
right. This entails responsibility.
Individual users are responsible for the calculator while in their possession. The calculator will be returned to the
library at the end of each semester in the same condition as when checked out. In addition, the student/parent shall
be held financially responsible should the calculator be lost, stolen or damaged.
Students are responsible for batteries.
Students must turn in the calculator assigned to them.
