Job Description:
 Receives the ward from outgoing nurse and makes actual rounds with the nurse paying
attention to new admissions patients for X-ray, patients for laboratory examinations and
for surgical operations.
 Checks medicines, treatments, injection cards and carries orders on a specific time.
 Admits and discharges patients; recording hospital routines and policies
 Gathers information on admission like proper identification and other circumstances.
 Makes accurate assessment of patient’s physical condition and takes note of relevant
signs and symptoms.
 Takes patient’s history, allergies and other manifestations in the family.
 Monitors vital signs and reporting of any untoward signs and symptoms.
 Checks and makes changes in the diet list to be submitted to the dietary department.
 Charts accurately all treatments, injections and other pertinent observations.
 Administering medicines conscientiously by observing the ten rights.
 Maintains cards and nursing supplies.
 Imparts the importance of health teachings to patients and their families.
 Assists the physician in bedside rounds and procedures.
 Puts patients at ease and reassuring him whenever necessary in giving treatment for his
comfort.
 Special procedures such as, but not limited to: IV insertion, IV re-insertion, nasogastric
tube insertion, nasogastric feeding and flushing, suctioning, oxygen saturation checking,
oxygen inhalation administration, Jackson-Pratt, T-tube, hemovac and penrose drain
output measurements, straight of Foley cath insertion, hemoglucotest montoring, wound
dressing, cleansing enema.
 Prepares the client scheduled for operation, such as skin preparation, vital signs taking,
removal of dentures, jewelries, and nail paints.
 Does post- operative care such as vital signs monitoring, proper postioning of the clients,
and observations as to itchiness, dyspnea, operative site pain, bleeding, dressing, tube
placements, urine output and IV fluids.
 Makes proper documentation of care given to patients.
