Commercial Heifer Certificate
Requirements for entering a Commercial Heifer:
1. Entry Deadline – August 25, 2014
a. Entry must be submitted online by August 25.
b.
Completed Commercial Heifer Certificate or Registration Papers and a right side profile picture of the
heifer must be uploaded online with the entry.
c.
Individual ID – ONE of the following is required: official USDA tag (orange or silver bangs tag), RFID
tag, or legible tattoo (unreadable tattoos will not be accepted). The ID number submitted must match the tag #
or tattoo in the heifer’s ear. Random spot checks will be made by the superintendent at the show.
d. Exhibitor must submit a Legal Release and entry fee payment. All required material should be mailed to the
Tulsa State Fair.

 Fill in the highlighted fields and save the completed document to your computer.
 Upload this document & right side profile picture of the heifer online when submitting online entries.
1. After you checkout your online entry cart, you will be directed to upload files for each heifer you entered.
To make sure you are entering the correct files for each heifer, compare the WEN# on the online receipt
to the WEN# on the upload website.
2. In the Entry Description box type in the heifer’s tag or tattoo #.
3. In the first box click “choose file” click on your saved ownership document then click “open” and “upload”
to upload the ownership document.
4. The next box, upload a picture, “choose file” select the picture and then click “open” and “upload”.
5. Click “Done” and then “Done” to confirm.

Agreement Statement:
I hereby attest that the heifer described above has been/will be in the sole ownership and continuous care of the exhibitor
from August 1, 2014 forward. I understand that this document, the heifer, and photo will be inspected by the commercial
heifer superintendent at time of check-in at the Tulsa State Fair. I accept responsibility for the accuracy of the information
and photo provided above. I understand that discrepancies between this information and the heifer presented at check-in
may result in disqualification from the show. Online submission of document requires that you agree and will abide with
the terms as defined in the published rules.
