Planning for 2015-2016
The time has come for Barbara Morgan S.T.E.M. Academy to begin planning for the space
and staff requirements for the 2015-2016 school year. To facilitate the planning process we need
a firm commitment from you. Due to a large number of requests for next year, we want to be
certain that we retain a seat for your child. This form is a commitment, and any changes have a
negative impact on class size and programs available.
You do not need to reapply, but we do need this form for our records. This form must be
returned no later than December 19, 2014 in order to confirm your child’s placement. This
form must be completed for each child in your family. You may return this form:
 To your child’s classroom teacher
 By post office mail
 Via school fax with signature
Your failure to return this form will forfeit your child’s placement in the School of Choice or
program. We require a signature and cannot accept phone confirmations. We only can accept email confirmations if this form is scanned first and contains a parent signature.

How to complete the General School of Choice Application
To complete an application for Barbara Morgan S.T.E.M. Academy, you must
go to:
 http://www.westada.org/Page/1125 (School of Choice application page)
 Scroll down screen and click on the gold “General School of Choice
Application”.
 Complete one application for each student that is applying to Barbara
Morgan S.T.E.M. Academy.
 An email confirmation will be sent to the email address that you provide for
each applicant.
 All applications must be received by 11:59 pm on Sunday, February 1.
Applications must be completed by the due date specified above in order to be
considered on time. If more students apply than seats are available at any School
of Choice in any grade level, students will be selected by random lottery.
Lotteries are held throughout February (see the 2015 - 2016 schedule below). The
order in which lotteries are drawn is randomly selected and changes
yearly. Applicants should be aware that they can only be drawn for ONE
lottery. Once an applicant is drawn in a lottery, they are removed from any
lotteries that are to follow. Applicants will be notified via email of the results of
the lottery on or shortly after February 24th.
