The next step is for you, the Agency Supervisor to complete the Final Placement Report in the last week
of the placement. Electronic forms in Sonia Online, and paperwork must be completed before the student
leaves the placement.
You will need to log into Sonia Online, go through the Final Placement Report with the student and
discuss your comments. Submit the form in Sonia Online.
The student will also present you with their Experience Log including total hours. Please check the Log
carefully to ensure that you agree that it is a true record of the activities completed on placement and
particularly of their counselling hours.
The student will then submit the Experience Log within one week of completing the placement. This is a
requirement of the placement and failure to submit within one week may result in the placement being assessed
as unsatisfactory. If you have any questions or concerns, please contact your Placement Coordinator.
