Purpose of Job
As directed by the Team Leader the post-holder will be an
integral member of a team delivering comprehensive support
packages of care to adult service users seeking help in
addressing their substance and social care needs. The postholder will carry a case load and act as the care navigator for
service users and families, providing comprehensive
assessments, care planning & reviews with an emphasis on
improving the well being and engagement of parents
accessing drug treatment.
A main purpose of the post will be to strengthen the
communication with key partners working in a collaborative
Hidden Harm & Think Family way and a no wrong door
approach for Service Users and Families seeking assistance.
Providing a range of recognised psycho-social interventions
the post holder will assertively follow up service users who
need additional support with issues surrounding parental
care and vulnerable adult issues with a key focus on the
think family agenda and hidden harm and support based on
building on personal assets & Recovery Capital. The post
holder will be expected to liaise closely with Peterborough
City Council, Social Care Staff and providers across the city
and develop positive working relationships.
In fulfilling their duties, the post-holder will be expected to
work with individuals to develop positive lifestyle changes
and health choices. Promote harm reduction and
personalised health promotion interventions and encourage
people to access mainstream services.
The focus of all interventions will be:

To reduce drug related harm to the individual,
children, families and wider community

To promote healthier life styles and increase service
user choice

To offer assertive follow up and increased retention

To promote and support individuals into both
abstinence based and medically assisted forms of
recovery

To provide effective case management and
co- ordination of the individual’s treatment plan

To promote carer and service user involvement
The post holder will also focus on early intervention,
assertive follow up work engaging Service Users who are
identified as needing additional support through home visits
and satellite work within Family Centres.
The post-holder will contribute to the service to ensure that
service users consistently receive high quality services that
adhere to the best practice guidelines and achieve high
professional standards.
They will be responsible for contributing to the overall
performance of the service to ensure that contractual output
targets are achieved. They will also be required to record
and input client data and information in order that the
service operates within contractual, administrative and
financial requirements.
The post holder will be required to work across a number of
operational sites across the area as required and work
flexibly to maintain the most appropriate level of service
provision. This may include evening working.
The post holder will be expected to take responsibility for
personal development, identifying personal training needs
and participate in regular supervision and appraisal.

Organizing and managing packages of support to enable service users to
lead the fullest lives possible;
Recommending and sometimes making decisions about the best course of
action for a particular service user and families; liaising with, and making
referrals to, other agencies;
Participating in multi-disciplinary teams and meetings, including
Safeguarding & mental health.
Maintaining accurate records and preparing reports for proceedings
Participating in training, supervision and team meetings
To maintain a client caseload with Care Co-ordination responsibility for
clients.
To provide recovery & family coaching using a broad range of psychosocial
interventions
To utilise motivational interviewing, ITEP, cognitive behavioural techniques
and brief interventions in both one-to-one and group settings to promote
engagement in treatment services, encourage self-esteem, well-being, selfresponsibility and enhanced motivation.
To deliver and supervise parenting support interventions and training










To assist and carry out appropriate screening processes including urine and
saliva tests.

To work in partnership with community Pharmacists ensuring clear
communication pathways in relation to medication and supervised
consumption.

To contribute to the development of individual community support plans with
those who express a wish to access community treatment/support services.

To develop and review care plans, provide health education especially in
regard to harm minimisation, blood borne viruses and overdose prevention.

To promote sexual health awareness and offer screening and materials to
service users.

To offer advice and support to clients who present for BBV screening, and
ensure appropriate referral to specialist services.

To take a proactive support to practice development, maintaining a current
knowledge of appropriate interventions with regard to substance misusing
clients & parenting and supporting individual and team learning and
development

To refer and liaise with other agencies involved with the client’s broader care
plan including health services, hospitals and GP’s.

Recognise personal and professional boundaries and work within CRI
clinical governance framework at all times.

Present a professional image and actively promote the service at all times to
service users, other stakeholders, partnership organisations and the general
public.

Actively participate in regular supervision and be willing to personally reflect
on interpersonal dynamics within the therapeutic relationship and provide
evidence of achieving performance targets alongside other supervision
requirements as detailed in CRI policy.

Work within CRI’s professional development framework participating in
active learning and developing professionally, in accordance with service
and organisational requirements and current skills and abilities.

Uphold data protection legislation and only share client information as per
CRI policy, local and formally agreed information sharing protocols and client
confidentiality agreement

Person Specification
Essential criteria:
 Experience of working with drug issues and a clear understanding of the need for
and ability to deliver quality services.
 Experience of Safeguarding and the think family agenda
 Qualification in substance misuse and / or relevant experience
 Knowledge of the issues facing drug users including appropriate medical
interventions, social care and health issues.
 Motivational interviewing techniques.
 Knowledge or experience of CBT, Relapse Prevention or Brief Solution focussed
Therapy
 Knowledge and experience of family systems therapy
 Ability to assess and care plan service users and formulate written reports as
necessary.
 Community based support practices.
 Maintain professional boundaries and work effectively with partnership agencies.
