Please complete the following for each of your assigned sub-domains:
Sub-domain: Population Based Care
Statements related to sub-domain:
Population Based Care influences and enhances Patient Centered Care through a process of critical
health assessments of similar groupings of patients and related populations. These populations may be
based in geographics, gender, health behavior choices, race, health status, and chronic disease to name
a few. Population Based Care influences individual healthcare through population health needs, health
status and culture of the patient.1,2
Population Based Care enhances Patient Centered Care through evaluation of interventions in context to
the above mentioned populations, developing “best practices” and “practice guidelines” in relation to
healthcare therapies.
The most pronounced benefit of Population Based Care goes beyond reactive health interventions by
addressing disease prevention and health promotion affecting groups and populations. Population
Based Care transcends individual medical treatment and targets underlying risks, such as tobacco, drug,
and alcohol use; diet and lifestyle; as well as culture, race and environmental factors. 2,3,4
Key terms and definitions:
Population Based Care - Focuses on entire populations and is grounded in an assessment of the
population’s health status. It considers the broad determinants of health, emphasizes all levels of
prevention, and Intervenes with communities, systems, individuals and families.5
Patient Centered Care - Health care that establishes a partnership among practitioners, patients, and
their families (when appropriate) to ensure that decisions respect patients’ wants, needs, and
preferences and that patients have the education and support they need to make decisions and
participate in their own care.6
Best practices - a technique or methodology that, through experience and research, has proven to
reliably lead to a desired result. A commitment to using the best practices in any field is a commitment
to using all the knowledge and technology at one's disposal to ensure success.7 Standard operating
procedures that are considered state-of-the-science consistent with all applicable ethical, legal, and
policy statutes, regulations, and guidelines. 8
Practice guidelines - systematically developed statements to assist practitioner and patient decisions
about appropriate health care for specific clinical circumstances" (Institute of Medicine, 1990). They
define the role of specific diagnostic and treatment modalities in the diagnosis and management of
patients. The statements contain recommendations that are based on evidence from a rigorous
systematic review and synthesis of the published medical literature.8
