ASSOCIATE TEACHER FINAL EVALUATION
DIRECTIONS: This evaluation should be completed, printed, signed by the cooperating teacher and the
principal, and returned to the Tchr Ed Dept within three days of the final day of Associate Teaching.
This Microsoft Word “form” should be completed on your computer. Type into the gray text boxes (which
will expand as you type) or place an X in a box. Move the cursor with the Tab key. If this process does
not work, follow these steps to be sure the form is “locked”: (1) Right click the toolbar area at the top
of your screen. (2) Select the “forms” toolbar. (3) Select the “Protect Form” icon (the padlock.)
ASSIGNING A FINAL GRADE: At the bottom of this page, the cooperating teacher should
recommend a final grade, after consultation with the college supervisor. Please review the information
concerning evaluation in the Handbook for Associate Teaching before completing the form.
SUMMATIVE COMMENTS: It is important to write a comprehensive summary (on the final page)
regarding the candidate’s performance. These comments become a permanent part of the candidate’s
credentials file and are frequently a decisive factor in a school’s decision to employ the candidate.
A well-written, comprehensive summary and/or letter of recommendation will be beneficial for your
associate teacher, who may be in competition with others for a teaching position. The following
quotations will illustrate the type of information that could be included.*
1. School Setting: “The modern, single story school building to which she was assigned has an ample
supply of instructional materials. Most of the children in the self-contained classroom are from middle
class homes in what was considered a model suburban community.”
2. The Specific Assignment: “During his twelve weeks, he gained experience teaching high school
swimming, basketball, golf, bowling, weight training and conditioning.” “She had complete
responsibility for the class for __ weeks, teaching all subjects, and taught reading and language arts for
___ weeks.”
3. Evidence of Extra Efforts: “He has shown willingness to participate in areas beyond required
obligations by assisting in sports and officiating at track meets.” “She evidenced pride and commitment
to teaching as a profession by attending faculty meetings, in-service programs, and scheduled activities
of the school in which her students were involved.”
4. Effectiveness and Future Potential: “He demonstrated throughout his assignment that he possesses
the qualities essential for success as a classroom teacher.” “Academically, the class progressed well
under her teaching. Scores on the unit test showed gains in knowledge of the U.S. Constitution.” “I have
been particularly pleased with the way he has worked with my remedial reading group.” “She has a
warm and pleasant way with children. She cares about them as individuals and children respond well to
her. She communicates effectively.”
DISCUSSING AND PROCESSING THE EVALUATION: If possible, please hold a conference with
the candidate to review the semester of work, discuss this final evaluation, and answer questions. After
it has been received in the Tchr Ed Dept office, the supervisor will add comments and discuss the report
with the candidate. The candidate’s signature on the form at that point indicates that he/she has
reviewed the report, but does not necessarily imply agreement with evaluation.
