2. Duly accomplished Personal Data Sheet with recent passport-sized picture (CSC Form 212 Revised 2005) which
can be downloaded at www.csc.gov.ph;
3. CSC Certificate of Eligibility (authenticated);
4. *Transcript of Records;
5. *Certificates of Seminars or Trainings attended;
6. *Service Records or certification from previous employers stating the position assumed and corresponding duties
and responsibilities;
7. Performance ratings in the last two (2) semesters (for government employees only).
