What is the
Intent to Apply Process?
S The intent to apply is your declaration to the Health
Professions Advising Office (HPAO) and the Health
Professions Committee (HPC) that you intend to apply to
health professional schools for this upcoming application
cycle.
S The application is 24 pages in length. Much of the
information that you must provide will also be useful as you
prepare your professional school application.
S The information in the intent to apply application assists the
committee in composing a letter on your behalf.

All LOE noted on your ItoA application (if you add more email us and updated
application with revisions)

Digital photograph (JPEG file-low resolution) (photo used on our congratulations
board once accepted to a professional school)

S
Provide us with an upload date once everything has been completed.
The complete ItoA application and process will be largely directed towards applicants
who are requesting a committee packet. A committee packet contains a committee
letter and all other letters of evaluation requested.

Committee Letter Eligibility
S Full-time or part-time matriculated CSUF students, alumni,
and non-matriculated students who have completed the
majority of prerequisite coursework at CSUF.
S UEE students registered for a minimum of 20 units at CSUF
and will provide an LOE from a CSUF.
S Submitted all of the required documents by 8 AM on
Monday, February 16, 2015.

S
Unofficial transcripts from all institutions attended. If courses from one institution
are also listed on the transcript of another institution, there is no need to submit
both transcripts. Please note, these transcripts are for internal purposes only and
cannot be forwarded to the centralized application service or the schools themselves.

At least (1) letter of evaluation from a health professional
S
Please note that a letter documenting a minimum of 50 hours in a Clinical Care
Extender (CCE) program or similar program can substitute for a health
professional’s letter of evaluation
S
Digital photograph (JPG file – low resolution)
S
Fee payment
File Fee
S $20 per professional program for current students and recent
alumni (graduated less than one year).
S $100 fee to apply to multiple professional programs for
alumni who graduated more than one ago.
S File fees can only be paid through the Health Professions
Advising website or at the Student Financial Services (SFS)
Window (UH-180).
S File fees are non-refundable and cannot be rolled over to
future ItoA application cycles.
IMPORTANT FEE NOTICE:
S The $20 fee per profession will provide one upload to a
professional school application service. A $25 fee is
required for any additional upload/mailing to a
Caribbean school. All other additional uploads which
may consist of uploading/mailing of a committee/letter
packet, update to an applicant’s list of schools, and/or
individual letter uploads, will require a fee of $20 each.
Please plan accordingly, as to which schools you would
like to apply to, as this will prevent additional upload
fees.

TITANium helps you digitally track your file’s progress. Students are
automatically added to the HPAO’s TITANium site in 1-3 business days
upon submission of the ItoA application.
S
Please pay special attention to the notes that the HPAO leaves for you on
TITANium as this is how you will track your letter status as well as any
missing items.
S
All necessary documents can be downloaded/uploaded from TITANium.
A quick tutorial on how to upload a document can be found on our
YouTube channel.
S
Please check your preferred email on a daily basis, as this is our main way of
communicating with our students.
Intent to Apply Application
S Please complete the entire application. If any area is left
blank, the application will be returned.
S The application is to help prepare you for your submissions
to AMCAS, AACOMAS, AADSAS, OPTOMCAS,
PHARMCAS, CASPSA, VMCAS, TMDSAS, AACPMAS
etc. and as such should be completed with the same amount
of care.
S At least one letter from a health professional is due before 8
A.M. on 2/16/2015.

Future Applicants
S Applications are accepted year-round for those who only
wish to have the HPAO collect letters of recommendation.
S Only required to complete pages 3-7 of the ItoA application
and last 3 pages (no fee required).
S The Intent to Apply file will close each November.
S Resubmit the application every year to keep your file active
and updated.
Rollover
S Applicants who do not receive admission or do not
complete the application process will be automatically rolled
over to the next ItoA cycle upon receipt of the Intent to
Apply Re-Applicant Form and all necessary documents.
Missed Deadline
S If you fail to submit or request all of the required items by
8am on February 16, 2015, you will not receive a committee
letter of evaluation for the upcoming application cycle.
S The office will however still upload your letters in the form
of a letter packet to the professional schools to which you
are applying.
New Updates
S New Fee Policy (reviewed earlier)
S UEE students who are a non-Pre-health Professions
Certificate student may request a committee letter but they
must complete 20 units and request an LOE from a CSUF
Faculty for their professional school packet
S Health Professions Scholarship- application only available to
students in current cycle.
Last Year Updates
S The Reference Request Form is now no longer required with
each letter of evaluation submitted. The form now serves as
a helpful guide thus it should still be passed along to all your
letter writers.
S The autobiography portion of the ItoA has been removed
and is no longer required
S Now in addition to institutional course GPA information we
now require all pre-requisite course grades

S Now a self assessment table has been added to the application
asking students to rate themselves in 8 different areas (ex.
Critical thinking, professionalism, etc.)
S An ethnicity and race identification page has now been added
to the application

file will not be shredded or disposed of until five years
time.
S After 5 years time only a digital copy of your file will be
kept
Remember
S There is no associated deadline for letter packets as they are
composed on an individual basis
S Please note that letter packets do not contain a committee
letter but rather are a compilation of all letters of evaluation
requested.
