NOTICE OF CHEMICAL PROCUREMENT
Please fill out this form anytime you intend on bringing a chemical into an Olin Laboratory. The Environmental
Health & Safety Office must obtain records and the Safety Data Sheets (SDS’s) of all of the chemicals in Olin
laboratory to comply with OSHA and other Federal Regulations. Please either forward this notice to the EH&S
Office via email bkautz@triumvirate.com or bring the completed form to the Campus Center, Suite 332.
Procedure for Ordering New Chemicals
1. Is the chemical really necessary?
a. Consult current chemical inventories.
b. Are there safer alternatives or other chemicals currently on site?
2. Complete the “Notice of Chemical Procurement” and forward it the Environmental Health and Safety
Office with an SDS.
3. The Environmental Health and Safety Office will review the request and quickly reply.
4. After approval from the Environmental Health and Safety Office, the chemical may be purchased.

Please include a copy of the SDS before ordering the chemical. A copy of the SDS must be kept in the lab(s) the
chemical is stored in and used. Another copy of the SDS must also be kept in the EH&S office (Campus Center
332).
