How To Create a Program
Activity And Add An
Instrument
On the home page click Evaluation and
select Program Activity.
On the top right side of the page click
Add.
On this page add the information for the class
that was taught.
When you are finished adding the information
click create at the bottom of the page.
A Program Activity has been created.
Click SNAP-Ed Custom Data.
Scroll to the bottom of the page and click the
Add button for SNAP-Ed Custom Data
Add the closest appropriate answers for the class that
was taught and click Create at the bottom of the page.
Click Demographics.
Fill in the demographics for your entire class as a group.
At the bottom of the page click Create.
Click Instruments.
At the bottom of the page click Add.
Choose Food $ense Class Participant Form. In the next box choose
Retrospective Survey. Fill in the date the class was held. The Name box
will auto populate. Click Add.
Click the icon on the lower left side.
Fill in the information from one Class
Participant Form.
At the bottom of the page click Add. A new blank Class
Participant Form will appear on the screen. Continue adding
participants. When all participants have been added click Go
Back to Program Activity.
Click Mark Program Activity as
Completed
The Home Screen will now show your completed
Program Activity with Instruments attached.
