Upon the recommendation of the Search Committee, I am pleased to recommend to the Chancellor and the Presidents that you
be appointed Dean of the (COLLEGE NAME) and Professor of (DISCIPLINE) with tenure.1 This is a 100% administrative, fiscal year
appointment. This offer is subject to the approval of the Presidents and Trustees of Indiana University and Purdue University. In your
case, we have every reason to believe that such action will be forthcoming. Employment in this position is contingent upon a satisfactory
background check. The initial salary for this position is ($ AMOUNT) for the fiscal year and as a new employee of IPFW you will be
paid by Purdue University. This salary includes an administrative supplement of ($ AMOUNT) for performing dean duties. Salary
increases will be contingent upon job performance reviews. In addition to your salary, Purdue provides a generous benefit package that
includes a defined contribution retirement plan as well as medical, disability and life insurance which will begin immediately upon your
employment. A more complete description of these benefits can be found at http://new.ipfw.edu/offices/hr/benefits/. In order to assist
you with your move to IPFW, you will be provided a moving allowance of ($ AMOUNT). This lump-sum payment represents taxable
income, and taxes will be withheld from the payment. If these funds are used for qualified moving expenses per current Internal Revenue
Service guidelines, income taxes withheld may be recovered on your tax return. Please consult your tax advisor. Please contact the
school’s Business Manager, (NAME) at (260) (PHONE NUMBER), for further details regarding your moving expenses.
Your appointment as Dean will begin on (DATE), with a comprehensive review of your performance in spring (YEAR) 2. In the event
that you no longer serve as Dean, the administrative supplement would be removed and your appointment would be changed to an
academic year basis by the standard conversion. You will receive a separate mailing about the New Faculty Orientation and you are
expected to attend.
Enclosed you will find President’s Office Form 19 “Purdue University Appointment to the Faculty” and "Notice of Terms of
Initial Appointment for Appointments to Indiana University Missions." To indicate your acceptance of this appointment, please sign
these forms, as well as this letter, and return them to me in the envelope provided. I will then forward them for final approval by the
Presidents and Trustees. The enclosed Employee Information, Self Identification Compliance, Background Check Form, Direct Deposit
Authorization, W4 and WH4 forms should also be completed and returned with your materials at this time. Please refer to Indiana
University's Academic Handbook, which can be found at https://www.indiana.edu/~vpfaa/academichandbook/index.php/Main_Page. It
details policies and procedures that apply to IPFW faculty appointed to Indiana University mission programs. Also for your reference
Purdue University’s Faculty and Staff Handbook which can be found online at http://www.purdue.edu/faculty_staff_handbook/. In
addition, I am sending you University policy I.A.1 and copies of Purdue executive memoranda (B4, B48 and B50) and OAA 05-2 which
are relevant to your employment. The policy and memoranda are for your permanent records.
Employment with the University is subject to the requirements of the Immigration Reform and Control Act (IRCA) which
requires employers to verify an employee’s right to work in the United States. Prior to your first day of work, you must complete Section
1 of the U.S. Citizenship and Immigration Services’ Form I-9, Employment Eligibility Verification. To access the electronic form, visit
the IPFW Human Resources site at http://new.ipfw.edu/hr/. Click the link titled, “Electronic I-9 for new hires/rehires,” login and
complete Section 1 of the Form I-9. You will then be provided a list of acceptable documents. The required documents must be
presented to your school’s business manager or Human Resources on or before your first day of employment.
