Current State of Colorado Registered Nurse License (R.N.)
EXPERIENCE:
1. One year experience with developmentally disabled adults in a community, private or
institutional setting.
2. Two years of management experience.
OTHER REQUIREMENTS:
1. A criminal and Motor Vehicle record which meets company standards.
2. A valid Colorado Driver’s license.
3. Verified legal status to work in the United States.
4. Ability to use computers fluently (Internet, Word Processing).
5. Exceptional oral and written communication skills.
6. Basic money management and budgeting skills.
7. Strong organizational skills and adept at performing multiple tasks at once.
PHYSICAL DEMANDS/WORKING CONDITIONS
Requires concentrated visual and auditory attention; requires frequent standing and walking;
lifting and transferring individuals, carrying, reaching, pushing, and pulling; ability to work in
environments that could include changes in temperature/humidity, dust, odors, noise, various
allergens (both chemical and food related) etc.; ability to work with individuals with disabilities,
a variety of illnesses, and medical conditions; working indoors, outdoors, and in vehicles; works
around and with others, and alone; may have to walk on slippery and /or uneven walking
surfaces; expected to assist individuals receiving services with entering and exiting the vehicle,
toileting, cleaning, bathing, cooking and other basic life skills.
POSITION SUMMARY
The Manager oversees and participates in the development and delivery of services designed to
further the abilities of adults with developmental disabilities to live, learn and work in a more
independent and inclusive manner. The Manager is responsible for direct support, administrative
duties and management of staff in the designated program.
ESSENTIAL FUNCTIONS
Administrative Duties
1. Responsible to assure all shifts are covered.
2. Complete all required assessments and reports in a timely manner.
3. Ensure that the program is staffed appropriately to meet individual needs and budgetary
requirements.

4. Provide input to the Finance Office for budget development.
5. Adhere to program budget. Minimize scheduling overtime and utilize resources
efficiently.
6. Verify and approve staff timekeeping records on a regular basis (no less than weekly).
7. Review and respond to leave requests.
Supervision of Staff
8. Provide leadership by demonstrating a strong work ethic, solution oriented thinking, open
mindedness, active listening, good judgment, decisiveness, accountability and strategic
thought.
9. Communicate expectations clearly and maintain a high level of accountability.
10. Train new hires on all job duties and ensure competence.
11. Recognize good performance frequently and specifically.
12. Provide constructive criticism privately and professionally.
13. Address performance issues in a timely and constructive manner and implement
corrective action or recommend termination as necessary.
14. Conduct staff meetings at least monthly; meet with individuals or small-groups as
necessary.
15. Notify Human Resources of staff vacancies. Interview applicants as requested.
16. Complete annual performance evaluations of staff.
17. Responsible for supervising, mentoring and training staff during all work hours.
Nursing
18. Provide regular and ongoing training to direct support staff in the care and use of
gastrostomy tubes and catheters in accordance with state regulations.
19. Document nursing notes for all assessments conducted.
20. Write and/or review all medical protocols.
21. Coordinate medical appointments and any follow-up, such as medical treatments and
recommendations, and monitor implementation.
22. Ensure appropriate documentation including medical occurrence forms, dictations, test
results, labs and orders are completed/obtained in timely manner and attached in Therap.
23. Provide after-hours medical advice to staff in the event of a medical/medication question,
treatment question, medical emergency or development of a serious new health problem
to determine treatment.
24. Exercise informed judgment and uses individual competence and qualifications as criteria
in seeking consultation, accepting responsibilities, and delegating nursing activity to
others.
25. Participates in activities that contribute to the ongoing development of the profession’s
body of knowledge.
Program Specific Duties
26. Plan healthy and varied menus for the house weekly. Assure menus meet the Department
of Health regulations.
27. Balance and reconcile client personal needs funds records and financial records on no
less than a weekly basis.
28. Safeguard individuals’ belongings and regularly update individual inventories.
29. Attend and assist clients at medical appointments as needed.
30. Ensure medications and medical supplies are on hand as ordered.

31. Respond to and correct deficiencies found during internal monitoring and external
surveys.
32. Submit monthly billing and attendance accurately and timely.
33. Approve MAR and perform medication audits monthly.
34. Each Manager will be assigned oversight of one or more functional areas including but
not limited to day services, residential services, recreation or greenhouse. Managers are
responsible to fulfill duties required for each position not enumerated in this job
description and to maintain the necessary expertise in his or her functional area.

35. Respect each individual’s rights within the program and treat all individuals with dignity
and respect at all times.
36. Report any incidents of suspected rights violations, mistreatment, abuse, neglect or
exploitation immediately according to MVDS policies and procedures.
37. Respect the values, opinions, preferences and choices of individuals.
Person Centered – Focusing on what is important to an individual receiving services
38. Educate and encourage individuals to make informed choices regarding their lives.
39. Provide opportunities and choices for growth and development based on individual
interests, preferences and input.
40. Ensure access to community resources based on individual interests, preferences and
input.
41. Encourage individuals to take ownership in maintaining their environment.
42. Provide emotional support, social guidance and positive role modeling.
Safety (Risk Management) – Protecting individuals from harm
43. Assist individuals to maintain a safe and clean environment.
44. Create and maintain program’s chore chart for a list of duties.
45. Provide safe transportation of persons receiving services, drive defensively and follow
traffic laws.
46. Follow safety guidelines including, but not limited to healthy lifting, transferring, and
back care.
47. Identify and solve potential safety hazards, repair or maintenance needs or immediately
report them to the Operations Manager.
48. In case of emergency, follow the emergency plan identified for the facility; notify
appropriate authorities and Mountain Valley staff as soon as possible.
49. Conduct, document and participate in emergency drills as required.
50. Ensure the vehicles are well-maintained and cared for.
51. Interact successfully with people in times of crisis or unpleasant environments.
Individualized Plan and Care Coordination
52. Participate in the development of the Individualized Plan (IP) for each resident including
analyzing progress and developing new goals for the biannual assessment.
53. Provide services and implement goals in accordance with the Individualized Plan for each
individual supported.
54. Communicate with the Interdisciplinary Team as needs change.
55. Assist individuals with physical and personal care as identified in the Individualized Plan
(IP).

56. Assist non-ambulatory individuals with physical transfer as necessary, including, but not
limited to, pushing wheelchairs, assisting someone in standing from a seated or laying
position and entering and exiting vehicles.
Documentation
57. Review necessary documentation at start of shift and enter necessary documentation at by
the end of shift per MVDS policies and procedures.
58. Respond to correspondence (emails, voicemails, Therap, etc.) in a timely manner.
59. Enter attendance and billing in a timely manner.
Medical Needs
60. Communicate with the Medical Office about observed changes in physical and emotional
needs in a timely manner.
61. Accurately administer medications, document administration and report/document all
medication administration errors, as directed by the Medical Office and medication
administration policies.
62. Follow health protocols and care plans.
63. Ensure documentation of all health information as needed (vitals, seizure reports etc.).
64. Prepare and serve balanced, healthy meals according to doctor ordered diets.
OTHER REQUIRED DUTIES
Standards of Behavior/Conduct
1. Respond to all requests for information or communication in a timely manner.
2. Report all on-the-job injuries to immediate supervisor and Director of Human Resources
within 24 hours of the occurrence and provide a detailed account of the events leading to
the injury.
Attendance and Scheduling
3. Report for work on-time and as scheduled.
4. Enter daily, accurate timekeeping records in timekeeping database.
5. Attend all regular staff meetings and other meetings as requested by supervisory or
administrative staff members.
6. Work schedule as assigned.
7. Have all time-off requests pre-approved.
8. Work at any MVDS facility and be responsible for own transportation to work.
Competencies and Certifications
9. Complete all mandatory new hire requirements and training as identified by the State of
Colorado and Mountain Valley’s training requirements within 90 days of hire and
maintain courses and certifications as required.
Adherence to policies and procedures
10. Adhere to MVDS policies and procedures outlined in the policy manual, Employee
Handbook, and other employee policies.
11. Complete other duties and responsibilities as assigned to ensure effective functioning of
Mountain Valley’s programming.
