Right person or patient.
Ask for identification such as name, DOB, or NHS number/wrist band.
2nd R?
Right drug.
Use the generic name, know the drugs action and rationalise its use, understand the common drug endings.
3rd R?
Right dose.
Understand units and standard abbreviations. Do not use non-standard abbreviations. Check dosage against JRCALC guidelines.
4th R?
Right time, day and frequency.
Check start date of drug. Check duration of prescription. Check if drug needs to be taken before/after eating. I.e PRN, OD, BD, TDS, QDS, etc
5th R?
Right route.
Is the drug IV, IM, Sub Cut or IO
6th R?
Right to refuse.
The patient has the right to refuse dispensation, however you may have to notify third-parties as necessary.
7th R?
Right documentation.
Sign the prescription chart if you are sure the patient has taken the drug. Write down the appropriate number if administration was not possible.
