SPRING SEMESTER 2015 (Term #1152) REGISTRATION GUIDELINES Classes Begin on Monday, January 12, 2015 (When registering, use the spring #1152 registration segment. Be certain to use the correct class numbers and double-check your schedule before or during the first week of classes when it is easiest to make schedule adjustments.) IMPORTANT: 1) Be certain to register for the required number of “units” to activate your tuition authorization. 2) Remember to click on the “GREEN” submit button to record your transaction before exiting your window. 3) Print a copy of your schedule after completion (for future reference) to verify registration. We’ve encountered some students who thought that their registration was complete only to later learn, through the Graduate School’s term audit, that there were no classes on their schedules. This results in petitions to the Graduate School to add the course/s and an extremely expensive penalty for the student for each course registration. “REGISTRATION AND FEES AT A GLANCE:” For complete registration dates/deadlines, please visit the Office of the University Registrar’s web site: http://registrar.osu.edu/registration/hints/RegGlance REGISTRATION UNITS: 1. Pre-candidacy: register for at least 8 credit units to activate your tuition authorization: you may register for as many as 18 credits. 2. Post-candidacy: register for 3 credit units each semester until graduation. 3. Fellowship/Trainees: must have 12 credit units on your schedule unless you have passed your candidacy; following candidacy, only 3 credit units are required. 4. Be certain to check your schedule during the first week of class when it is easiest to make any changes. AUDIT units do not count toward the total unit requirement to activate tuition authorizations. REMEMBER: “always add a course before dropping one” to avoid any forfeiture fees (explained below). PENALTIES: (Please see the Graduate School “Spring 2015 Semester Deadline” information to be posted on our web site upon receipt): 1. “Try” to register by 5:00 p.m. on Friday, January 2 – We are asking that all your registration be completed as soon as possible, but no later than Monday, January 5 (earlier is better to insure the posting of your tuition) well in advance of the onset of spring semester. 2. Students are responsible for payment of any late registration or late tuition payment penalties! 3. Penalties of $200.00 for late tuition payment will be assessed to accounts from January 6-23 for currently enrolled students. Please review the deadline/penalty information carefully. 4. FORFEITURE FEES: If you are making a schedule change and your units drop below the required number of units for enrollment status, e.g., a post-candidacy student dropping from 3 to 2 and precandidates dropping below 8, you will be charged forfeiture fees according to the number of units below your requirement. This is the reason we ask that you to always ADD a course before DROPPING one. You must always maintain the required number of credit units for your registration category (see above: Registration Units). 5. You may add a course “on line” through the first Friday of the semester only. For spring, this would be January 16. You may add with permission from January 23. Starting on January 24, you will need to use a “Late Add Petition Form.” You may drop on line through Friday, February 6. From February 7-March 27, you must drop using a “Change Ticket” with the signature of your advisor and processed at the Graduate School by one of their registration personnel. 6. After March 27, a “W” (for withdrawing after the posted deadline) will appear on your permanent transcript. 7. Dropping a course between the 4th and 7th Fridays requires a “Course Enrollment Permission Form” with the “drop the course” in the lower right of the form check marked. This form is available in my office or on the Graduate School’s web: http://www.gradsch.ohio-state.edu This form will require the signatures of your instructor, advisor, and department chair (the chair of the course that is petitioned to be dropped). 8. The last day to register for the “Audit” option is Friday, February 6. STUDENT HEALTH INSURANCE The department will continue to cover 85% of the health insurance with the remaining 15% deducted monthly from your stipend checks. Please be reminded that if you do not maintain a full-term appointment, you will be charged for tuition and student-related fees. In addition, you will be charged not only for the remaining university’s portion of the student health insurance subsidized premium, but also the 15% for which you are responsible. Student health insurance is not automatically cancelled with an appointment termination date. GRADUATION 1. End-of-Semester Autumn 2014 Graduates: All documentation must be in the Graduate School no later than 5:00 p.m., Friday, January 9, 2015. This includes your final oral exam report form, and the dissertation approval form. You will receive your diploma at the spring commencement on Sunday, May 10, 2015. 2. Spring Semester 2015 Graduates: If you are planning to graduate in spring semester, May 10 commencement, or for end-of-semester spring (degree conferred at the summer commencement on August 9, 2015), your Application to Graduate form must be submitted to the Graduate School by Friday, January 30. Applications will now be submitted electronically: please visit: GRADFORMS.OSU.EDU to initiate your application to graduate in the spring semester (or end-ofspring semester). 3. All Ph.D. processing fees ($45) are now automatically added to your student account when you submit your Application to Graduate. This is a one-time fee applied to your account about the fourth week of the semester. 4. The fee will be carried on the account until the graduation semester, but is due the term you apply to graduate or your enrollment registration will be locked for the next semester/term. 5. Master thesis students are no longer required to pay the $12 library processing fee. 6. Doctoral hoods: students will purchase the doctoral hood directly from the OSU Bookstore at the same time arrangements are made for the tam (cap) and gown. The hoods will still be delivered at commencement and presented to the students as part of the ceremony. Students will pick up the tam and gown before commencement at the OSU Bookstore. CONTRACTS If there is a change in your contract for the spring semester, I will notify you personally via email. Please keep in mind that when you sign our contract, it is your “promise/pledge” to be here during the dates of your appointment: FOR TAs: the service is from the “day prior to the onset of the semester” to “the last day of final exams for the semester . . . or mutual agreement with the faculty instructor.” FOR FELLOWSHIP and RESEARCH: the service is from the “day prior to onset of semester” to “” . . . ‘mutual agreement with (faculty instructors) and/or advisor, or graduation’ . . . “ IMPORTANT: There may be financial consequences for late arrival (after the beginning of the semester) or for leaving before the end of the semester, depending on the circumstances of the absence. This may result in a deduction in stipend. Please keep in mind that if you have a TA appointment, you MUST have a replacement AND have faculty approval before leaving.
