HOSA: National Service Project Guidelines Cystic Fibrosis Foundation (CFF) The HOSA National Service Project involves the sponsorship of a national healthcare organization by local HOSA chapters. Local chapters plan service projects to support the selected organization. National HOSA’s chosen service project is the Cystic Fibrosis Foundation (CFF). Chapters document their involvement and submit documentation to their state advisor by the state deadline to be eligible for national recognition. For specific details, refer to National Service Project guidelines. RULES FOR ELIGIABILTY: 1. For recognition at the HOSA National Leadership Conference, documentation of National Service Project awards must be mailed by the State Advisor and received in the National HOSA office by May 15. 2. At the National Conference, participating chapters can be eligible for two different awards. All requirements for each award must be met in order to be eligible for award. Certificate of Recognition: This award is presented to chapters who have either donated up to $100 OR have volunteered up to 100 hours total for the CFF. Certificate of Merits: This award is presented to chapters who have either donated up to $500 OR have volunteered up to 500 community service hours for CFF. PROCEDURE: 1. Prepare/ Host Fundraising Event. (Hawaii HOSA highly suggests doing the “65 Roses Fundraiser.”) 2. Collect/ Record all hours or donations and fill out the HOSA EVENT REPORT (LINK to form is at the end of this packet). Make 3 copies of report. 3. Send money and (1) copy of the HOSA EVENT REPORT to address for the San Diego Office of the CFF listed below in #7. Our contact person is Stasha Ybarra. If you have any questions, you can email her at sybarra@cff.com. 4. Another copy of the HOSA EVENT REPORT must be sent to Hawaii HOSA’s state office by February 10, 2013. Please be sure to also attach either a letter or receipt from the CFF acknowledging your contributions. For without a letter or a receipt, the chapter will not be eligible for awards or recognitions. 5. The last copy of the report is kept at school for personal records. 6. State Office must then submit forms and names to National HOSA BEFORE MAY 15th. 7.
