Application
Introduction
This application for United Way of Greater St. Louis MAP funding has been developed to help member agencies
present their funding request in a concise format to United Way volunteer reviewers. This information will be
used to evaluate the merits of each request, including the clarity and effectiveness of design, success in
implementation, and proven impact. Funding will be considered for programs that demonstrate these
characteristics, but are also competitive, clearly identify a community need and address United Way of Greater
St. Louis priority areas.

Please Note:
 Agencies interested in applying for funds must complete the following application and email one copy to
reporting@stl.unitedway.org by 5:00pm on September 15, 2015. Late applications will not be accepted.

Agencies are limited to one application.

Agencies may request funding in the amount of $25,000 or $50,000. No other amounts will be
considered.

Eligible requests include programs or services that will be provided in the United Way of Greater St.
Louis’ 16 county region in the following categories:
o New - not currently in operation at the agency and will not begin until after funding is awarded.
o Existing - currently in operation at the agency and will continue as is with no significant changes.
o Existing with enhancement or expansion - currently in operation but significant changes planned.

Capital requests will not be considered.

Please note character limits provided include spaces. As a guide, 500 characters = approximately 100
words.

Keep responses concise and use at least 10 pt. type.

Handwritten forms will not be accepted.

Ensure all fields are complete. Incomplete applications will not be reviewed by the volunteer committee.


Some of the questions are similar to those used during the United Way of Greater St. Louis reporting process.
You may choose to use data or responses submitted during reporting, but it is strongly advised that you assess
the responses and augment them as needed to provide the strongest application possible.
This is a highly competitive application process and we want you to have the best opportunity possible to be
selected. Your responses should be clear to the average person who does not have in-depth knowledge of your
area of work.

Explain why the program is needed, how you determined the need, and the scope of the need. If you cannot
find local statistics, please include the most current data available. Local data is strongly preferred. Identify
who you intend to serve – include specific demographics such as their age, where they live, etc. as is
pertinent to distinguish your request.
2. Describe if this request will meet the needs of an underserved population or fill a gap in
services. Why is your agency best positioned to meet this need? (2,500 character limit)
We consider underserved as someone having difficulty obtaining help due to lack of awareness, availability,
accessibility, affordability or another barrier. In addition to overall agency competencies and a summary of
capacity, you may include agency accomplishments, specialized training, or other supportive evidence
indicating why your agency is in a strong position to meet this need. List relevant certifications, trainings,
awards, recognitions, or accreditations.

Request Overview
3. Describe the program or service that you would like funded, and why you believe this should be
selected for United Way investment. If it is an enhancement or expansion, describe proposed
changes and why they are deemed necessary. (3,500 character limit)
Include detailed description, where and when the services take place, number of sessions, on-site or off-site,
etc. If enhancement or expansion funds are requested, describe specific changes that will occur if funding is
awarded and why they are needed.
4. Describe how this request will create new or enhance current relationships/collaborations that result
in people being helped more efficiently or effectively. (2,500 character limit)
List the collaborative partners with regard to this program and how they help you achieve positive client
outcomes. Give examples of current or anticipated efficiencies.
If there are no collaborations at this time, state “Not applicable.”
5. Discuss whether this effort should be considered innovative or if it uses evidence-based practices.
(2,500 character limit)
Innovation is defined as using advanced, forward thinking, or original methods and can apply to things such
as the program model, means of implementation, location of implementation, target population served, or
staff position created. Highlight what sets this approach apart from programs serving a similar population.
Evidence-based practices have a published research base that verifies the efficacy of the intervention with
specific populations. If evidence-based practices are used, details regarding the evidence used are required.
Include why this particular model is a good fit for your agency.
If you do not consider this request innovative or evidence-based practices are not used, state “Not
applicable.”

Specifically include the sources of data, frequency and method of collection, use of software/database, and
use of validated, reliable measurement tools if available. Explain how benchmarks were selected. Share
results from the last year if available. Describe how use of this process will provide quality outcome data.
Include the protocol for how your agency evaluates feedback data to enhance, change, or improve this
program. Provide examples of how feedback from people you support and other data from performance
management methods are currently used to improve results.
8. Describe your current efforts in relation to the requested program or service, including challenges

Provide details regarding agency efforts implementing this program or programming that is similar. Have
there been any surprises? What has worked better than expected? What have you learned? What has
been modified due to previous results?

The ‘Agency Budget’ is identical to the one used during United Way reporting. Use the budget submitted
during reporting to complete this question. Do NOT add funds requested in this application.
B. ‘Program Budget Proposal’
Enter the proposed budget for the requested program only, NOT the agency’s entire program budget.
This includes requests for new, existing, or existing with enhancement or expansion funding.
Include funds requested in this application.
Example:
Proposed program budget = $150,000
Agency is submitting MAP application for $50,000 from United Way.
Enter budget for entire $150,000, which includes the $50,000 being requested in this application.
C. ‘Budget for New UW Funds $25,000’
All agencies will complete this section. Enter budget for funds requested in this application that total
$25,000.
D. ‘Budget for New UW Funds $50,000’
Complete this section if you are requesting $50,000.
If you are requesting $50,000 you must enter the budget for BOTH $25,000 in section "C" and $50,000 in
section "D" in the event volunteers have chosen to invest in your request at a lower level.
In section 'D' enter budget for funds requested in this application that total $50,000.
10. Provide a narrative justification for the proposed budget, describing specifically how the funds will
be used to achieve outcome indicators. (2,500 character limit)
Use this space to clarify what you have entered in the budget. It should be clear exactly how the additional
United Way of Greater St. Louis funds will be used. Include information regarding other funding sources
currently received or anticipated that will support the program.
