I understand that she must abide by the rules and regulations set forth by the coaches and the principal of Fairhope Middle School. I understand that my child and at least one parent/guardian must attend the Try-out meeting on Thursday, February 12, 2015 @ 6:00 and that ALL required forms must be turned in by February 27th or my child will not be allowed to try out for cheerleading. I understand that my child must attend the Try-out Clinic to be considered for a position on the squad. I understand that qualified judges will evaluate my daughter, and we agree to abide by the decision of the judges. I understand the financial obligations involved if my child is selected to the squad. I understand the very nature of the activity; cheerleading and gymnastics carry a risk of physical injury. No matter how careful the participant and the coach are, how many spotters are used, or what landing surface is used, the risk cannot be eliminated. I understand the risk and will not hold Fairhope Middle School or any of its personnel responsible in the case of accident or injury at any time. Student Signature: Date: ____________ Please review the 2015-2016 FMS Cheering Guidelines on the FMS website fairhopems.com 2015-2016 FMS Cheer Try-Out Clinic/Try-out Information Cheer Clinic: Monday, March 9 - Thursday, March 12- 4:00-5:30 @ FMS Gym Over the course of 4 days, you will learn the jumps, cheer, and dance that you will perform at try-outs. You will receive the dance music via email after the 1st day of clinic so that you can practice at home. Cheer Clinic Attire -Any athletic style top/t-shirt (no midriffs) and any athletic shorts -Appropriate tennis shoes with no show socks -Hair in a neat pony tail- no ribbons -No jewelry Requirements/Percentages for Try-outs: During the 4-day cheer clinic, you will learn a cheer, jumps, spirit and a dance routine. On the day of try-outs, you will perform those skills and be evaluated by 3 judges. School attendance, teacher recommendations and discipline will also be considered. 15% Tumbling- Running (5%) /Standing (10%) 20% Jumps-Toe touch & Front Hurdler 20% Group cheer 20% Dance 15% Overall Effectiveness(Motions/Spirit/Voice) 10% Teacher Evaluations, Discipline, Attendance Tryout Attire -White athletic style tank top (no midriffs) and Black athletic style shorts (Reasonable length) -Appropriate tennis shoes with no show socks -Hair in a neat pony tail- no ribbons -No jewelry Reminders for clinic & try-out: Be on time & prepared No cell phones Bring bottled water No parents please Bring a positive attitude! Financial Obligations-if selected for the 15-16 FMS Cheer Squad $200 will be due at Uniform/CampWear fitting (April 14th) $65 per month- Squad Tumbling- not paid through school account The following is an ESTIMATE of the cost that will occur with cheerleading. Fundraising opportunities will be available, but may not cover the entire balance.
