Appealing for readmission Policy Students dismissed from EOPS due to back-toback semesters of non-compliance may submit a written request for readmission. (No second or status downgrades appeals.) Procedure Review appeals process with EOPS staff Complete 6 units with term GPA of 2.0 or better semester before appealing Submit written appeal with supporting documents Submit EOPS application Using book vouchers (paperless) Policy 1. Student must meet with ALL instructors during office hours to obtain instructor feedback for ALL enrolled classes. Must obtain feedback from ALL instructors. 2. Procedure • Download APR at EOPS website (forms tab) • Make appointment with instructor • Submit APR to EOPS Office by October 2, 2015, 1:00pm Requesting a nine (9) unit waiver Policy 1.
