The Philosophy of the Continuum

Our aim is to provide the supervision needed to keep each individual safe and to provide the supports needed to keep each individual moving towards a less restrictive environment.

Transitional goals are an important part of each Individual

Habilitation Plan and are focused on acquiring the skills needed for increased independence and employment.

Transition Planning

 Parents can begin this process by contacting the local

Social Security office (1-800-772-1213 or http://www.ssa.gov/

).

Medicaid

 Students should apply for Medicaid at age 18.

 Medicaid will act as a secondary insurance if the student is covered by their parent’s health insurance.

 Medicaid one of the eligibility requirements for DDD services.
