Practice Placement Time Log Form
Please complete this form and submit it to the School Office (Health Sciences) at the end of
semester 1 & 2 .assessment tutorial. It should be countersigned by your PPE. Please keep a copy
for your Practice Placement Portfolio.
Practice Placement Log Forms need to be completed and presented at the end of semesters 1 and
2 assessment tutorials to your Personal Academic Tutor. The student has to make sure that these
forms are completed and signed by the PPE. Any remaining days that are still to be undertaken on
Practice Placement after the assessment tutorial in semester 1 or 2 needs to be discussed as
supervision would need to remain in place.
