Addition of Personnel to IACUC Protocol Dear Principal Investigator: Please complete the attached Addition of Personnel form for each individual you will add to your protocol(s). All personnel working with animals in research must file a Training Summary form with the IACUC office. Please contact the IACUC office PRIOR to adding personnel to your protocol to ensure proper documentation is on file. It is mandatory for all personnel (including Principal Investigators) to have completed training in: laws and regulations pertaining to the care an use of animals in research and species-specific educational courses through the Collaborative Institutional Training Initiative (CITI), hosted by the University of Miami. In addition, SUNY Upstate Medical University’s Research Integrity Office requires research ethics education for all basic science & clinical investigators (including faculty, postdoctoral fellows/associates, clinical residents/fellows, students, lab staff, research nurses, clinical research associates and individuals from affiliated Universities participating in SUNY Upstate research projects).
