OESP Parent Survey (REAL)
Dear Parents/Guardians:
Thank you for giving your time and knowledge to help us learn
more about your child with a disability and his or her experiences with online education.
For
this survey, online learning is defined as formal education where a student learns through the
Internet. This can include both self-paced and instructor-led activities. Online learning can be
either fully online or blended (a combination of online and face-to-face).
Please share
information related to your child with a disability who is receiving at least a portion of his or her
education in an online environment. If you have more than one child in an online environment,
please use information from the same child throughout the survey.
Completion of the
survey indicates your willingness to participate in this project and that you are over the age of
eighteen.
