University of Alabama School of Medicine Mandatory Requirements To Be Completed Prior to Matriculation Class of 2018 Check when complete The following requirements must be completed by July 14, 2014. Although certain websites listed below may have a completion date later than July 14, 2014, you must meet the School of Medicine’s July 14, 2014 deadline. If accepted after July 14, 2014, the following requirements must be completed within 2 weeks of your acceptance. Requirement Official Final Transcript with Degree Posted – A paper transcript must be sent from your school’s Registrar to UASOM. BlazerID UAB Email Account HIPAA Privacy and Security Training Instructions Contact Information and Web Address Must be received by Admissions office no later than July 14, 2014. This ID is required to access various UAB & SOM services & websites, and is required before some of the items on this checklist can be completed. See guidelines concerning choosing a professional BlazerID. Establish a UAB email address. Please wait at least 24 hours after creating your BlazerID to complete HIPAA training. Access HIPAA training in UAB Learning Management System (LMS). Before any training module or requirement can be accessed via the LMS, the student must be entered into Banner. Once that is completed, a new student must create a BlazerID and password. If the student already has a BlazerID and password, but is newly associated with a HIPAA covered entity, time will be required for the HIPAA training to appear in his/her assignment list. The process from being entered into Banner to creating BlazerID and password to having the required UAB/UABHS HIPAA Privacy and Security Training appear in “Assigned Learning” within the LMS could be up to 48 hours. Go to the link:www.uab.edu/learningsystem Select Learning System (LMS) login and log in using your BlazerID and password Click to place the radial button under “Student/Manager” and Select “Proceed with Login” “UAB/UABHS HIPAA Privacy and Security Training” will appear under “My Learning” Select “UAB/UABHS HIPAA Privacy and Security Online Training”. Please make sure to choose/enroll in the ONLINE version, NOT the classroom version. An option to print a certificate is displayed in the green banner (this certificate is available to be printed at any time by accessing “My Transcript,” selecting the HIPAA course, and a green banner for printing a certificate displays) An evaluation of the course is available yet optional Successful completion (a score of 75% or better) will automatically be recorded by the LMS; if unsuccessful, repeat the steps listed Department of Veteran Affairs New Trainee Orientation Print and complete all required documents on the VA Web site. Print and follow the TMS Instructions for Self Registration then complete the mandatory training, print the certification of completion, sign all forms and be fingerprinted. Student Health Services (SHS) Medical Clearance – Step 1: Obtaining Information and Completing Required Forms Step 2: Submitting Documents to CertifiedBackgroun d.com The Medical Clearance process will be accomplished through a webbased program. The service is provided by CertifiedBackground.com, a secure, confidential, web-based company that is HIPAA/FERPA compliant. They will provide an Immunization Tracker and Certified Record Review. STEP 1: Enrolling students must show proof of immunizations with either an official certificate of immunization, a photocopy of an immunization certificate, or written documentation from their physician. The immunizations needed and the form to complete can be found at http://www.uab.edu/studenthealth/medicalclearance/dentistry-medicine-optometry. Students must provide proof of their immunizations and TB testing, Clinical Immunization Record, and Clinical Student Health History. *PLEASE NOTE: Students can prepare this documentation NOW to upload to “CertifiedBackground.com” after 6/11/ 2014. Dental Insurance You will be charged a dental health fee unless you provide proof of dental insurance. To request a waiver fax a copy your insurance card to Dorothy Andrews at 205-934-3057.
