Vision: Making the Healthy Choice the Easy Choice
Mission: Creating Healthy Communities (CHC) is committed to preventing and reducing
chronic disease statewide. Through cross-sector collaboration, we are activating communities to
improve access to and affordability of healthy food, increase opportunities for physical activity,
and assure tobacco-free living where Ohioans live, work and play. By implementing sustainable
evidence-based strategies, CHC is creating a culture of health.
In attendance were: Mel Milliron, Patrick Peddicord, Jack Roberts, Chris Kotapish, Kim
Mascarella, Marilyn Walton, Marie Economos, Sheila Calko, William Young, Carol Beltz
Review of meeting activities: The following is a summary of each agenda item as discussed
during the meeting.
2015 Creating Healthy Communities Overview
 We have three priority communities: Bolindale, North Warren (north of Market and
west of Elm Road, South warren (south of Market and west of Rt. 169)
 We must complete the CDC CHANGE tool by June.
 We have one coalition objective. We have a great coalition so this one was difficult. We
will do work to help all of our agencies to better understand and practice population
based/evidence based interventions that focus on policy/system/environmental change.
 One Active Living objective per community
 One Healthy Eating objective per community
 One Tobacco Free Living objective per community
Active Living
 In Warren North we will partner with The Child Care Connection and our Help Me Grow
staff to train early childcare centers in the Ohio Healthy Program.
 In Warren South we will partner with Trumbull Neighborhood partnership to build a
pocket park near the bike trail.
 In Bolindale we will partner with Howland Township to repair Bolindale Park. This is a 2
year project

Healthy Eating
 In Warren North Comprehensive School Health will be th focus. OSU Extension will do
a Smarter Lunchroom intervention at McGuffy. We will have School Food Service
Conference, a School to Farm Road Trip and a “Water, First for Thirst” campaign.
School Food Service and Farm to School conferences will be open to all schools that
want to attend.
 In Warren South SCOPE will partner with us on a Mobile Food Pantry. SCOPE has
already located people in 200 homes who have no means of transportation or way to get
to food pantries.
 In Bolindale Howland will offer a Double-up program at their farmer’s market
Tobacco Free Living
 This was a difficult one to choose. We had the option of Tobacco Free School Campus,
Tobacco Free open spaces (parks) or Smoke-Free multi-unit housing.
 We chose Smoke-Free multi-unit housing. We will develop a 2 year campaign to help
multi-unit housing facilities become smoke free.
CHANGE
 We spent good deal of time developing a strategy for doing the CHANGE Tool.
 Kent State advanced nursing students are working on the Warren City school portion.
 We identified potential locations.
North Warren Worksites include: AVI McDonalds Corporate, Delphi, City and County
Government, The Tribune, Giant Eagle, Warren Fab, Thomas Steel
North Warren Community Institution/Organization (CIO)’s include: YMCA,
YWCA, Someplace Safe, Family Mission, Tod Baptist Church, Victory Christian
Church, Eastern Gateway Community College
South Warren Worksites include: TCAP, Coke plant, Loy McCoy, Craft Maid,
Trumbull Memorial
South Warren CIO’s: WIC, Grace AME Church, Loyd McCoy,
Bolindale Worksites include: Patcatan’s, Ridge Plaza stores, Nursing Home, Bowling
Alley, Fair Haven
Bolindale CIO’s: Trumbull Business College, Bolindale Churches,

