Power Plant Valve Repair Services
1.0 Introduction
Colorado Springs Utilities is seeking contractor(s) to provide supervision, labor,
materials, equipment, supplies, and facilities as required, to provide Power Plant
Valve Repair Services as more fully described in this Statement of Work,
(“Work”).
For the purpose of this document, Colorado Springs Utilities will be referred to as
(“UTILITIES”). UTILITIES’ Project Manager will be referred to as (“PM”).
Contractor, individual(s), or companies responding to this solicitation shall be
referred to as (“CONTRACTOR”).
This Statement of Work will be used to establish multiple Blanket Contracts
for valve repair services for UTILITIES’ Power Plants. In addition, this will
also be used to award a contract for the Front Range Plant 2012 Fall
Outage.
The result of this solicitation will be a two-year contract with two (2), one-year
renewal options to be exercised as mutually agreed upon in writing.
Actual requirements may vary during the term of the contract and UTILITIES
does not guarantee a minimum total purchase. CONTRACTOR shall provide the
equipment and personnel necessary to handle the needs of UTILITIES.
LOCATIONS:
The Martin Drake Power Plant is located in the City of Colorado Springs at exit
141, off of I-25. The plant has an elevation of 5,975 feet above sea level. The
proper mailing address for the plant is: 700 Conejos Street, Colorado Springs,
CO 80903.
The Ray Nixon Power Plant is located in the City of Fountain at Exit 125, off of I25. The plant has an elevation of 5,570 feet above sea level. The proper mailing
address for the plant is: 6598 Ray Nixon Road, Fountain, CO 80817.
The Front Range Power Plant is located in the City of Fountain at Exit 125, off of
I-25. The plant has an elevation of 5,580 feet above sea level. The proper
mailing address for the plant is: 6615 Generation Drive, Fountain, CO 80817
The Birdsall Power Plant is located in the City of Colorado Springs. The proper
address is: 213 Nichols Boulevard, Colorado Springs, Colorado 80907.
2.0 Scope & Background
2.1 Scope
CONTRACTOR shall provide valve inspection and repair services for the
Front Range Power Plant 2012 Fall Outage and as requested by other
UTILITIES’ power plants. Valves can include, but is not limited to: safety

relief valves, process control regulating valves, block valves, root valves or
any other valve(s) identified by UTILITIES.
Required valve Work normally falls into the category of leakage past the
seat. Valves, which exhibit seat leakage, shall be repaired in accordance
with the manufacturer’s recommended repair instructions. Lacking
manufacturer’s instructions, the repair shall be “industry recommended
practice” and shall be discussed with and approved by the UTILITIES’ PM.
If new parts are required, UTILITIES reserves the right to procure those
parts for CONTRACTOR to install.
Upon each instance, UTILITIES shall notify CONTRACTOR and advise
CONTRACTOR of the services required, schedule, and any additional
information required. CONTRACTOR shall provide a quote, schedule and
any additional information needed. Should CONTRACTOR be unable to
meet any of the requirements or if the cost or timing is unacceptable,
UTILITIES reserves the right to contact another contractor to obtain their
services.
2.2 Background (Unit Descriptions)
Martin Drake
There are three units at the Martin Drake Power Plant for which UTILITIES
will require services. They are described as Unit 5, Unit 6, and Unit 7. Below
are the descriptions of each.
The Unit 5 boiler is a Riley Stoker, indoor installation, maximum continuous
capacity 425,000 lbs. steam/hr unit. Type of furnace operation – negative
draft, drum design pressure 1450 psi, non-reheat, tangential tube (nonmembrane) boiler with an operating pressure at superheater outlet of 1300
psi, and steam temperature at superheater outlet of 950 deg F. The
National Code Number is 2132 and the Riley boiler serial number is 3405.
The unit has been in service since 1960 and provides steam to a 44 MW
(nameplate, currently running 50 MW gross) Westinghouse steam turbine.
The Unit 6 boiler is a pressure-fired (positive pressure) Babcock and Wilcox
(B&W), Stirling Power (S-10191) design, indoor installation, maximum
continuous capacity 685,000 lbs/hr superheated steam at 1325 psig and
950 ºF unit. The unit has three B&W EL-70 pulverizers used to deliver coal
to the furnace. The fuel and combustion air enter the furnace through nine
burners arranged in three rows across the front wall. The type of furnace
operation – pressurized, non-reheat, bent tube, two drum design. The boiler
provides steam to a 66 MW (nameplate, currently running 84 MW gross)
Westinghouse steam turbine. The boiler has been in service since October,

1968.
The Unit 7 boiler is a B&W, indoor installation, maximum continuous
capacity 900,000 lbs. steam/hr unit. The type of furnace operation –
pressurized, reheat, bent tube, drum design, rated at 1005 ºF and 1600 psi.
The boiler provides steam to a 127 MW General Electric steam turbine
(nameplate, currently running at 141 MW gross). The boiler has been in
service since 1974.
Nixon
The Ray D. Nixon Power Plant is located in Fountain, Colorado. The Unit #1
Boiler is a Babcock and Wilcox RB-562, coal fired, radiant type, rated at
1,450,000 lbm/hr of steam, at 1005 F and 1990 psig. The unit has been in
service since 1980 and has approximately 230,000 operating hours.
Front Range
Front Range Power Plant is located adjacent to Ray D. Nixon Power Plant
in Fountain, Colorado. Front Range is a 480 MW combined cycle, natural
gas fired plant. The unit is comprised of two 130 MW GE PG7241FA
combustion turbine generators, two Tri-pressure Vogt NEM Heat Recovery
Steam Generators (with single reheat), and a 220 MW Alstom steam
turbine. Front Range utilizes an air-cooled condenser.
Birdsall
The Birdsall Power Plant is located in the City of Colorado Springs. The
Birdsall Power Plant has three (3) units. Two (2) units are Springfield
boilers, manufactured in 1951, with a total heating surface of 22030 sq. ft.
The third unit is a Springfield boiler, manufactured in 1955, with a total
heating surface of 26,659 sq ft.
3.0 Tasks
CONTRACTOR shall provide all materials and workmanship required to perform
Power Plant Valve Repair Services. CONTRACTOR shall provide the following,
but is not limited to: equipment, tools, consumables, storage, and transportation,
to meet the requirements of the contract, even though not specifically mentioned
in this Statement of Work.
Valve Work may be completed on-site or at the CONTRACTOR’S facility, as is
determined to be the most timely and practical for efficient repairs by UTILITIES.
All materials and every process and operation of valve inspection, disassembly,
reassembly, repair, test and replacement of various parts and materials shall be
subject to inspection by UTILITIES’ PM. UTILITIES’ PM shall have free access to

all operations and all parts of the Work at all times whether at or away from the
site. All costs associated with Work requiring repeated inspection because of the
rejection of unsatisfactory Work and materials, shall be paid for by
CONTRACTOR. Work found not acceptable by the UTILITIES’ PM shall not be
approved and shall be corrected by CONTRACTOR at no additional cost to
UTILITIES before Work is deemed complete.
Materials used in Work shall have approval by UTILITIES’ PM prior to
commencement of Work. All parts and/or materials provided by the
CONTRACTOR to be used in Work will meet or exceed Original Equipment
Manufacturers (“OEM”) specifications.
In the event, it is determined during valve maintenance, a particular valve
requires replacement, CONTRACTOR shall identify potential sources, costs and
delivery schedule. Based on CONTRACTOR findings, UTILITIES’ PM will make
the decision to either replace the valve or reuse it.
Insulation and lagging removal will be done by a separate contractor provided by
UTILITIES. Any insulation removal requests shall be coordinated through the
PM.
UTILITIES will provide scaffolding installation/removal as required for the valve
repair tasks. The area(s) requiring scaffolding and/or insulation work will be
identified prior to starting valve repair Work. All scaffolding and/or insulation
needs will be deemed necessary by UTILITIES’ PM. All scaffolding and
insulation requirements will be coordinated through the PM.
3.1 Front Range Power Plant Fall 2012 Outage Tasks
Pre-outage mobilization, scheduling, and staging
The following activities provide general guidelines for CONTRACTOR prior
to the start of Work. This list is not all inclusive. CONTRACTOR shall
include mobilization activities in Work Schedule.
CONTRACTOR staging shall include:

Set up storage space for tools and equipment

Set up office trailer (if required)

Inventory and procure material

Set up barriers (where required)
3.2 Valve Repair Services
CONTRACTOR shall provide valve repair services to include, but are not
limited to: inspection, disassembly, reassembly, repair, test and
replacement parts and materials. All repairs and parts and materials shall

be according to the manufacturer’s operating standards. All tasks in this
section are referenced in Exhibit A-1, Valve List.
CONTRACTOR shall provide all equipment, tools, labor and materials, as
required, to repair the identified valves.
3.3
Valve Repair Services for Boiler Safeties for Front Range
Power Fall 2012 Outage:
CONTRACTOR shall provide valve repair services to repair, test and certify
the boiler safeties. These safeties shall include the superheat and drum
(North and South).
CONTRACTOR shall have certifications required to meet ASME 52.01-120,
Boiler Safety Valves.
CONTRACTOR shall provide, but is not limited to: equipment, tools, labor
and materials as required, to repair, test and certify the boiler safeties.
3.4 Demobilization
CONTRACTOR shall remove all tools, equipment and material from the
plant site within 30 days of the end of the outage or Work.
CONTRACTOR shall remove from the plant all temporary storage areas,
job boxes, tool trailers and office trailers. Work shall not be considered
complete until all CONTRACTOR’s equipment has been removed from the
plant.
The UTILITIES’ PM will conduct a job-site walk-down that will identify any
areas requiring further clean-up. CONTRACTOR shall not release
employees and fully demobilize until PM is satisfied with cleanliness of all
work areas.
4.0 Schedule
The Front Range Power Plant outage will begin on or around September 22,
2012 and complete scheduled outage on or around November 6, 2012.
CONTRACTOR shall commence valve Work when the outage begins.
CONTRACTOR shall complete all valve Work no later than November 6, 2012,
unless CONTRACTOR is required for unit startup. Work may continue after this
time on equipment not critical for the start-up of the unit. All Work shall be
approved and coordinated with PM.
CONTRACTOR personnel shall remain on site until all Work is complete and
CONTRACTOR has received approval from UTILITIES’ PM to begin releasing
personnel and/or demobilizing.

CONTRACTOR shall provide a schedule to the UTILITIES’ PM within three
weeks of the award of Contract.
If CONTRACTOR begins to fail to perform under the Contract, including but not
limited to schedule, CONTRACTOR shall take all measures required to perform
Work per the Contract documents. This requirement is inclusive of all
CONTRACTOR personnel and CONTRACTOR’s subcontractor personnel.
The UTILITIES scheduled work hours for outages will be Monday through
Saturday, from 0630 to 1700 hours. If required, CONTRACTOR may work hours
outside of these mentioned, only with prior approval from PM. If there is a
requirement to amend the working hours once the project has started,
CONTRACTOR shall present the PM with the requirement, with justification. The
requested hours shall not be changed until the PM has approved the revision.
5.0 Additional Requirements for Blanket Contract
CONTRACTOR shall be a National Board Certified VR Stamp Shop, Section VIII
and Section I and authorized Dresser Green Tag Center. CONTRACTOR shall
be able to provide UTILITIES with the following services, but is not limited to:
 Manual valve repairs (gates, globes, checks, butterfly’s, plug, diaphragms
etc.)
 Actuation repair and calibration (hydraulic, pneumatic, electric)
 In-line safety valve testing and repair
 Control valve repair and calibration
 High pressure packing extraction, re-packing and lubrication
 Steam trap repair and sales
 Ball valve repair and automation (including ball-grinding capabilities)
 Regulator repairs
 Authorized Consolidated valve assembler
 In-line valve machining capabilities
 Machine shop capabilities or resources to provide machining services
 Pressure seals
CONTRACTOR shall confirm experience with several different valve
manufacturers. In many cases factory training is utilized to perform high level
repair Work. In other cases manufacturer’s audits or previous experience will
suffice to gain this authorization. Some manufacturers in the industry with which

CONTRACTOR shall demonstrate the required qualifications as outlined. If
CONTRACTOR decides to subcontract any of the items outlined in this section,
they shall provide a list of subcontractors that would be used to complete the
Work. All subcontractors are subject to UTILITIES’ approval.
Any equipment used by CONTRACTOR to move materials in and out of the
building shall be the responsibility of the CONTRACTOR to reserve, rent, lease
or buy.
CONTRACTOR shall certify that all Work is performed using industry standard
practices. UTILITIES’ PM will inspect all installations and material as deemed
necessary.
6.0 Deliverables
CONTRACTOR shall provide a detailed, type written valve inspection and repair
report at the completion of all Work to the UTILITIES’ PM. The report shall be
provided no later than two (2) weeks after the completion of Work.

CONTRACTOR shall provide the report in two (2) color copies, in a binder and
one (1) color copy on CD in Microsoft Word format.
This report shall include the following information:
 Valve number, manufacturer, size, type
 As found condition, with pictures
 Recommended repair
 Parts and/or materials used for repair
 As complete condition, with pictures
 Recommendations for future repair or replacement
 All testing datasheets, including completed hydroset data sheets
7.0 Additional Work
CONTRACTOR shall be responsible for submitting to UTILITIES all cost and
time estimates prior to beginning any additional work. All cost estimates shall be
a not to exceed estimate. It will be at UTILITIES’ discretion if additional work will
be completed based on the cost estimate presented by CONTRACTOR. No
additional work shall be performed without prior signed Plant Change Directives.
8.0 CONTRACTOR’s Requirements
CONTRACTOR shall provide industry qualified personnel and supervision to
complete planned and additional work scope.
CONTRACTOR is responsible for all permits necessary for the project and all the
environmental requirements stated by local and federal laws.
CONTRACTOR shall supervise, inspect and direct the Work competently and
efficiently, devoting such attention and applying such skills and expertise as may
be necessary to perform the Work in accordance with the Contract documents.
CONTRACTOR shall inspect and ensure quality control of all Work. UTILITIES
will inspect and confirm the quality control of all Work.
Upon award of the Contract, CONTRACTOR shall immediately begin to
schedule and prepare for Work required under the Contract. CONTRACTOR
shall place orders for equipment and materials required for this Contract scope in
a timely manner necessary to meet any time constraints of this Statement of
Work. CONTRACTOR shall furnish a schedule prior to the prior to the beginning
of the Work, which contains anticipated start and completion dates for tasks and
may include interim milestones when applicable to be approved in writing by the
PM.
CONTRACTOR’s work forces will NOT be permitted to use any power plant
facilities, equipment, or vehicles. CONTRACTOR shall provide its own office
trailer, tool trailer, lockers (or other storage facility) and lunch area.

CONTRACTOR will be permitted to store tools, equipment and material on the
job site and shall be assigned storage areas based on need and priority of work.
CONTRACTOR shall submit a request for storage area to the PM in advance of
the date the space is needed and shall be responsible for the security of all such
materials. UTILITIES’ does not take responsibility for any material or equipment
left on site during this project. CONTRACTOR has a duty to be responsible for
their own property that may be left on any UTILITIES’ job site.
Other work, by other contractors, will be performed during outages, so
CONTRACTOR shall coordinate with other contractors through the PM and daily
outage meetings. The daily outage meetings will occur at 0930 hours and a
representative of the CONTRACTOR is mandated to attend.
The CONTRACTOR shall prepare daily progress reports on the UTILITIES’
provided form. These reports shall include an updated schedule and any action
items identified from the past or current shifts. The report shall also include status
of subcontractors, materials, and other work. These reports will be provided to
the PM before the daily outage meetings and summarized at the daily outage
meeting.
9.0 UTILITIES’ Resource Requirements
Drake Power Plant:
 Primary industrial power is 480 volt, three-phase and is available in
limited locations.
 The 120 volt power outlets are available throughout the plant.
 Plant’s station air delivers 90 psig of air at various locations throughout
the plant.
Nixon Power Plant:
 Primary industrial power is 480 volt, three-phase and is available in
limited locations.
 The 120 volt power outlets are available throughout the plant.
 Plant’s station air delivers 90 psig of air at various locations throughout
the plant.
Front Range Power Plant:
 Primary industrial power is 480 volt 3 phase 100 amp service in limited
areas
 The 120 volt power outlets are available throughout the plant.
 Plant’s station air delivers 105 psig of air at various locations
throughout the plant.

Birdsall Power Plant:
 Primary industrial power is 480 volt, three-phase and is available in
limited locations.
 The 120 volt power outlets are available throughout the plant.
 Plant’s station air delivers 90 psig of air at various locations throughout
the plant.
The power and air hook-up requests from CONTRACTOR shall be submitted to
the PM, PRIOR TO THE BEGINNING OF THE PROJECT. A limit on power and
air usage may be enforced to ensure the safe operation of the power plant.
CONTRACTOR may be responsible to procure an alternative source of power or
air. A water source will be discussed as needed.
UTILITIES will provide sanitary facilities (port-a-pots) for all contract personnel on
the plant site with at least one designated for female use only. UTILITIES will
provide dumpsters unless otherwise stated.
10.0 CONTRACTOR’s Personnel and Subcontractors
10.1 Key Personnel
Certain CONTRACTOR personnel are considered essential to the Work
being performed and as such shall be referred to as Key Personnel
hereunder, and to such Key Personnel the following provisions will apply:
CONTRACTOR shall, at all times, provide Key Personnel of the
appropriate experience, qualifications, and knowledge to perform the Work
as specified in this Statement of Work and the manufacturer
specifications.
Prior to diverting any of the specified Key Personnel to other programs or
positions, or substituting any Key Personnel, CONTRACTOR shall notify
the PM in writing as soon as reasonably possible with proposed
substitutions including their experience and background as related to this
type of Work. Replacements or substitutions of Key Personnel shall not
be permitted until CONTRACTOR has received UTILITIES’ written
approval of the replacement or substitution.
CONTRACTOR shall provide a list of Key Personnel as required by
UTILITIES. UTILITIES reserves the right to view and approve/disapprove
any Key Personnel prior to start of project.
10.2 CONTRACTOR Personnel
If CONTRACTOR personnel lack the appropriate qualifications for this
Work, CONTRACTOR shall immediately replace that individual with
another employee with qualifications appropriate to the Work being
performed.

UTILITIES may also require that CONTRACTOR remove from the job, at
no additional cost to UTILITIES, employees who endanger persons or
property, are disruptive to the workforce, or whose continued employment
under this Contract is inconsistent with the requirements of the Contract
and/or interests of safety or security at the UTILITIES’ power plant.
Replacement of personnel shall be at CONTRACTOR’s expense and any
delay occasioned by the replacement process shall not entitle
CONTRACTOR to an adjustment in favor of CONTRACTOR.
11.0 UTILITIES’ Key Personnel
Authority of UTILITIES’ key personnel is as follow:
 PM: Responsible for the technical aspects of the performance of the
Contract. The PM will be the primary contact with CONTRACTOR’s team.
The PM may designate other personnel to oversee the performance of
Work, sign field tickets, etc. However, the designated PM retains ultimate
authority over the technical aspects of Work on each project. Should
CONTRACTOR and the PM disagree over the technical requirements of
the Contract such matters will be immediately referred to UTILITIES’
Technical Services Superintendent and/or Maintenance Superintendent
for resolution. The PM does not possess any authority, express or
implied, to direct CONTRACTOR to deviate from the Terms and
Conditions of the Contract.

Procurement and Contract Services (“PCS”) Representative: Responsible
for negotiating all terms and conditions of the Contract, coordinates prebid meetings and will be CONTRACTOR’s primary point of contact
through the solicitation and evaluation process. Additionally, all
correspondence shall be issued and received by the designated PCS
Representative. The PCS Representative is the only individual authorized
to direct CONTRACTOR to deviate from the express, written terms of the
Contract. All authorizations to deviate from the Contract must be
authorized in writing and signed by UTILITIES’ PCS Representative and
CONTRACTOR’s designated representative.

Safety Representative: All UTILITIES’ employees have the right to STOP
CONTRACTOR’s Work if unsafe work practices are observed. UTILITIES’
Safety Representative will monitor CONTRACTOR’s Work and may
request plans from CONTRACTOR for major lifts or other activities
requiring detailed safety plans as deemed necessary by UTILITIES.

The PM may furnish drawings and documents as necessary to identify
equipment locations and work areas prior to the Work commencement. All
drawings and documents are considered property of UTILITIES. Any and all
information used in connection with this project shall not be disclosed without
prior, written authorization from UTILITIES. CONTRACTOR shall be responsible
for said documents.
13.0 Safety
Safety is an extremely important value of UTILITIES and all efforts in regard to
UTILITIES’ safety and CONTRACTOR’s safety are expected. If CONTRACTOR
has more than twenty (20) employees on site, CONTRACTOR shall name a
safety representative who will be present while CONTRACTOR’s work force is on
site. CONTRACTOR will ensure all personnel conform to all industry, OSHA, and
UTILITIES safety guidelines and standards for safety. CONTRACTOR is
required to meet or exceed all OSHA regulations. Failure to comply with OSHA
regulations may result in removal from the jobsite and subsequent
disqualification from performing any work at any UTILITIES’ site.
A UTILITIES’ power plant safety orientation lasting approximately one (1) hour
will be administered to all CONTRACTOR’s personnel and subcontractors prior
to starting Work on site. At the completion of the safety orientation, all personnel
will receive stickers indicating completion of the orientation that must be worn on
their hard hats at all times while on site. Safety orientations will be given daily at
7:30 a.m. or on an as needed basis. If CONTRACTOR has personnel or
subcontractors who do not speak and read English, CONTRACTOR shall provide
translation during the safety orientation and while CONTRACTOR’s personnel or
subcontractor who do not speak and read English is on site, CONTRACTOR
shall provide at least one person to be available to translate at all times.
Hot work permits, confined space entry permits, electrical lock out and tag out
equipment clearance requests are required on the plant site. The request for
these shall be handled through the PM with a twenty-four (24) notice. UTILITIES
requires a fire watch person any time hot work could be present on the project
site. CONTRACTOR shall be responsible for the fire watch person to be present
one (1) hour after hot work is complete. CONTRACTOR shall not begin any
Work until all permits have been obtained. This Work is in a coal/ash dust
environment. CONTRACTOR shall be responsible for any wash down of area
before Work can begin.
CONTRACTOR is required to notify the PM of all incidents (as described below),
including minor incidents (within 24 hours), occurring in the course of performing
any UTILITIES’ projects on UTILITIES’ premises. CONTRACTOR will use the

STATEMENT OF WORK
Power Plant Valve Repair Services
Incident shall be defined as: An unexpected or unwanted change from normal
system behavior which causes or has the potential to cause personal harm,
property or environmental damage and/or loss of resources. An undesired event
that did or could have resulted in personal harm or property damage; any
undesired loss of resources.
CONTRACTOR must complete an incident investigation in accordance with the
CONTRACTOR’s written policy to identify the cause(s) and any action needed to
prevent recurrence. Joint incident investigations with the CONTRACTOR’s
Safety Representative may be required when deemed necessary by UTILITIES’
representative(s).
Hazards the CONTRACTOR identifies which are within the control of UTILITIES
should be reported to the PM immediately. These hazards will be investigated
and corrective measures taken, when appropriate.
The PM requires notification immediately (no later than 3 hours) following any
incident resulting in the following:

Death

Hospitalization (CONTRACTOR employee, subcontractor, public,
Etc.)

Damage to any vehicle which renders it inoperable

Amputation of any body part

Unconscious employee

OSHA Inspection with citations

Safety system failures (slings breaking, support structures collapse,
Etc.)

Near miss that could have reasonably resulted in any of the above.

Inspections
14.0 Personal Protective Equipment (“PPE”)
All employees/individual(s) on the plant site shall wear sturdy steel toe work
boots or metal tarsal guards based on site specific duties where injury could be
reasonably expected to the foot, OSHA acceptable hard hats, and safety glasses
with side shields while on site. UTILITIES recommends dust masks (if
applicable). CONTRACTOR shall provide CONTRACTOR’s personnel with all
appropriate PPE including, but not limited to; fall protection, monitoring
equipment, etc. All PPE must be worn at all times while on the UTILITIES’
power plant work site. Respiratory protection may be required for certain work in
UTILITIES' facilities. CONTRACTOR must implement the provisions of the
OSHA Respiratory Protection Standard 29 CFR 1910.134 when work requires
the use of respiratory protection.

15.0 Smoking
All areas inside the plant are designated as NON-SMOKING. CONTRACTOR
shall enforce this restriction. The PM will provide designated smoking areas.
These areas will be the only places smoking will be permitted for
CONTRACTOR’s personnel. Failure to adhere to this area will result in
revocation of all site smoking.
16.0 Work Site Clean Up
CONTRACTOR is required to clean up its work areas and maintain clean work
areas at all times. At the daily completion of the job and prior to leaving the
jobsite, CONTRACTOR shall clean up the work site of all debris left by
CONTRACTOR’s work force to as-found or better condition.
CONTRACTOR shall eliminate hazards such as power cords, welding leads,
torch hoses, etc. per OSHA standards. Cords shall be routed and protected in a
manner that eliminates safety hazards.
CONTRACTOR shall remove and properly dispose of all identified damaged or
replaced equipment and/or materials in the UTILITIES’ provided receptacles as
directed by the PM.
Before CONTRACTOR leaves the power plant work site at end of the project, a
CONTRACTOR’s representative must walk down entire job site with the PM or
the PM appointee to confirm that clean up is complete. Clean up shall include but
not be limited to all scrap materials generated and used by CONTRACTOR such
as insulation, welding rod, sheet metal, blast media, dirt, paint, refractory, plastic,
paper, plywood, steel, wood planking, card board, wire, pad eyes and other
rigging points, nails, fasteners, pallets, buckets, etc. Also trash generated by
CONTRACTOR such as drinking water bottles, paper cups, dust masks, soda
cans, tin foil, paper bags, gloves, cigarette butts, plastic wrapping, ear plugs, light
bulbs, boxes, cartons, packaging, etc., must be picked up and discarded in
supplied dumpsters. Clean up must be complete before Contract can be
considered fulfilled.
CONTRACTOR shall act responsibly with waste and / or removed materials. If
materials are temporarily staged to be discarded later, CONTRACTOR shall
follow up by moving these materials to appropriate UTILITIES’ provided
dumpsters before leaving site.
All scrap material is considered UTILITIES’ property unless otherwise previously
arranged through the PM, or is otherwise identified in this Contract.
Any markings made by CONTRACTOR for construction, repair, demo, install, or
temporary relocation, shall be removed, covered or made to blend in with

surroundings. These markings shall include paint, tape, markers, grease pencils,
chalk, etc. The PM shall approve final clean up of markings.
17.0 Invoicing
CONTRACTOR’s invoicing must match the pricing as provided in the Contract
documents. Any discrepant invoices will delay payment. Work is not considered
complete and payment will not be made until all deliverables have been met and
all final reports have been submitted.
All invoices must contain project location, PM name, PO/Contract number,
and if applicable, a Task Order Number.
To expedite payment under this Contract, send all original invoices directly to the
accounts payable office.

18.0 Security Requirements
CONTRACTOR shall provide to the PM, prior to commencement of the project, a
listing of all employees, including names, birthdates, and the type of United
States (“US”) government issued identification number that will be provided to
gain access to the plant site during the project. This list must be updated daily as
needed during the project. This list shall include the names of all subcontractor
employees employed by CONTRACTOR.
Names shall be submitted to the PM prior to any new employee’s scheduled
arrival. Failure to submit names to UTILITIES in a timely manner will result in a
delay of employee(s) access to site. UTILITIES will not be held responsible for
such delays. US government issued identification (i.e. driver’s license, passport,
etc.) will be required for plant site entrance. CONTRACTOR shall provide their
employees with labeling on their hard hats that state the company name and the
employee’s name.
CONTRACTOR’s assigned security representative or onsite project manager,
who will assume this role, shall promptly report all incidents involving the loss,
theft or vandalism of tools, equipment or material to the PM. Notification shall

also include incidents of violence, threats against others, illegal activities and
misconduct which is subject to disciplinary actions. Local law enforcement may
be contacted if needed. All reported incidents will be followed up by a written
report within 24 hours of verbal notification.
19.0 Prohibited Articles
Any person who possesses, transports, or uses prohibited articles on any
UTILITIES’ site is subject to immediate site removal. Prohibited articles include
but are not limited to:
 Dangerous weapons
 Explosives, ammunition, and incendiary devices
 Controlled substances and drug paraphernalia (e.g., illegal drugs and
associated paraphernalia, but not prescription medication)
 Alcoholic beverages (includes “near” and “non-alcoholic” beer and wine)
 Contraband (includes other items prohibited by law)
CONTRACTOR’s employees and employees of its subcontractors discovered on
the site in possession of any prohibited article will have the prohibited article
confiscated and the PM will be notified. If the prohibited article is illegal, local law
enforcement will be contacted. In all cases, the person in possession of a
prohibited article is subject to disciplinary action review to include access denial
to the site.
20.0 Right to Search
All vehicles entering and exiting the site are subject to search by UTILITIES. All
personal containers, lunch boxes, brief cases, etc. are subject to search at the
vehicle security access gates. Failure to submit to required search is in violation
of site work rules and grounds for permanent site access denial.
