The approach
Early involvement of parents in TAC discussions and goal setting
over time
Less than expected progress – TAC meeting agrees, with parents,
request for statutory EHC needs assessment – request
completed and sent to SNAS
If request agreed – plan co-ordinator allocated
TAC meetings (led by Lead Professional) co-produce plan content
including outcomes and consideration of personal budget
Draft plan returned to SNAS for decision
Follow up TAC to confirm implementation and review
arrangements
