1. What occurs if the game ends in a tie?
Answer:
If an exhibition game ends with a tie, then it is a tie unless, if it is a playoff tournament
game, there will be a 5-minute overtime period to determine the winner.
2. Name and describe each position in Floor Hockey.
Answer:
Coach – Also known as a head coach or an assistant coach who is given the task in
organizing the offensive and defensive players
Captain – A player who wears the “C” on his or her jersey, which signifies that they are
the leader of the team
Assistant Captain – A player or sometimes two players that wear the letter “A” on his or
her jersey, which signifies they assist or help the captain in running the team. Assistant
Captains takes the captain’s role if the captain is off the floor
Forward – A player who primarily plays offensive. There are three forwards, a left
winger, a right winger, and a center
3. Discuss and explain 5 different forms of penalties that occur in a Floor Hockey game.
Answer:
1. Holding – A form of penalty where a player intentionally grabs or holds the opposing
player’s stick or jersey
2. Tripping – A form of penalty where a player trips an opposing player by using their
stick or foot
3. Roughing – A form of penalty where two opposing players have an exchange which
usually results in some form of fighting
4. Charging – A form of penalty when a player takes more than three steps toward their

Answer:
A player properly holds their stick and instead of making a triangle, has their feet planted,
toes facing to the side and their hips facing the net. The player then holds the stick with
the blade behind the ball, and using their wrist and the end point of the blade, whipping
the ball towards the goal.
5. What is a face-off or 3 slap method and how do you perform one during a Floor
Hockey game?
Answer:
Two players from opposing teams must face each other slightly bent, properly holding
the stick. They then do the three slap method.
The three slap method is where the players will lightly smack each other’s shaft of the
stick three times then after, swipe the ball to gain possession.

Rationale on My Assessment
To start, for my Floor Hockey Unit I chose to use a Traditional Assessment. The type of
Traditional Assessment that I chose is a Constructed Response which requires more
thinking from the students where they have to fill in the answers themselves. Examples of
constructed response type exams are short answer, concept mapping, fill in the blank, or
essay questions.
For the first half of the exam, I have the students demonstrate their knowledge on Floor
Hockey by answering 5 short answer questions which are worth 2 points each (10 points
total). The reason why I chose short answer for the beginning of the exam is because I
want the students to show me that they comprehend the concepts of Floor Hockey. I felt
that the questions were developmentally appropriate for the students because they
correspond to what I wanted them to understand from the unit such as rules, equipment,
cues in shooting, positions, game, etc. Lastly, I will refer to my “Analytic Rubric for
Short Answer Questions” which is broken into three levels, depending on how well the
student’s performed in answering each of the questions. A two consisted that the student
fully answered the question correctly; a one meant the student answered the question but
were somewhat accurate; and a zero if the student was completely inaccurate or didn’t
answer the question at all.
The second half of the exam consist of a crossword puzzle where the students have to fill
in the blanks by using the definition clues on the bottom corresponding by their numbers
and whether the answer goes up or down. If each question is answered correctly, each
squared box will be filled in correctly as well, but if the answers are wrong, than the
crossword puzzle will not be completed correctly. Each filled in answer is worth 1 point
(15 points total). Again, I will refer to my “Analytic Rubric for Crossword Puzzle
Questions” which is broken down into 2 levels. To get a one, the student has to fill in the
squares completely and accurately. To get a zero, the student filled in the square
inaccurately or not at all. I will administer this exam on the last day of the Floor Hockey
unit and will inform the students a week before, so they can have time to study for it.
