Essential information Part 1 –SSN, US Citizen, Email, County School System. *If there’s no county listed, remember to complete a Form 4B *If there was a name change, please provide a copy of your marriage certificate, etc. Part 2 – Please answer all questions openly and honestly. If the applicant answers “yes” to any question, they must also include a detailed narrative of any/all incidents, any court and/or personnel documents, and documentation of completion of required programs or paid fines/fees. Hold a valid/unexpired teacher certificate/license issued by another state 2. Form 2 Documentation Needed for the Following Form 2 Form 2 Original Renewal 1. Endorsements for longbearing term ---minimum 12 transcript credit hours for each OR reflected on transcript 2. 12 clock hours of training Example of documentation of renewal hours for substitute Form 2 First Class Permit A first class permit (FCP) or out-of-field authorization (OOF) may be issued when an educator who is not certified for a specific employment position is hired by the county into the position, when no fully-certified educator has applied. First Class Permit Five Year Limit. –The position held is not subject to posting provided the educator meets the annual renewal requirements and completes the state approved program within five years. The State Superintendent may extend the five-year limit, if extenuating circumstances warrant the extension. First Class Permit Initial First Class Permit: The teacher certification specialist at the appropriate college/university must verify on the Form 1/1A application that 25% of the state-approved license program for the endorsement area has been completed. If not yet completed, the county superintendent may choose to submit a waiver request with the Form 1/1A application. First Class Permit For educators already holding an appropriate Professional Certificate, if the 25% program completion requirement for a First Class Permit has not yet been met, the educator may be employed on an Out-of-Field Authorization (OOF) until the 25% program completion requirement is met. First Class Permit First Class Permit Renewal: Six (6) appropriate renewal hours verified by the college/university must be completed each year for permit renewal. An official transcript must be submitted with each Form 1/1A. Both the college/university and the county must complete & sign each Form 1/1A. Form 1/1A Form 1/1A Form 1/1A First Class Permit First Class Permit/Out-of-Field Authorization: Form 1/1A Policy 5202 §126-136-11.1. http://wvde.state.wv.us/certification/forms/ Form 4 Professional Certificate Renewal, Conversion, or Permanent for Teachers and Student Support Form 4 Student Support includes the following: Counselor School Nurse School Psychologist Social Services and Attendance Speech Language Pathologist Speech Assistant **Student Support staff are exempt from the Beginning Educator Internship requirement Form 4 Professional Certificate Renewal for Teachers and Student Support If you are applying for a 3-year renewal, you must complete the following on Form 4: Part 1 – Check RENEWAL Part 2 – If you select Option 1 or Option 3, you must also complete Part 3 ( the original signature/date of the recommending college/university) and submit the official seal-bearing transcripts reflecting your 6 hours for renewal. --If you select Option 2, three (3) of your college credit hours must be in instructional technology per Policy 5202 Policy 5202 Instructional Technology According to Policy 5202, Section 10.1.4. B “Three of the six semester hours must be a course related to the improvement of instruction through the use of instructional technologies” Policy 5202 Instructional Technology • • • • This coursework should address: instructional technology operations/concepts the plan and design of effective instruction supported by technology the implementation of curriculum plans that include instructional technologies application of technology to assess and evaluate student learning Policy 5202 Recognition of College Credit for Renewal and Salary Purposes Section 9.6.6 Only unduplicated coursework related to the public school program as defined in 126-1369.6.7, completed through an accredited institution of higher education, as defined in 126-136-4.5, and subsequent to the issuance of the license being renewed and within the five-year period immediately preceding the date of application may be used for renewal of a license. Form 4 Professional Certificate Conversion, for Teachers and Student Support If you are applying for a 5-year conversion, you must complete the following on Form 4: Part 1 –Check CONVERSION Part 2 – If you select Option 1 or Option 3, you must also complete Part 3 (the original signature/date of the recommending college/university) and submit the official seal-bearing transcripts reflecting your 6 hours for renewal. --If you select Option 2, three (3) of your college credit hours must be in instructional technology per Policy 5202 Continued… Form 4 Professional Certificate Conversion, for Teachers/ Student Support Part 4 – Provide the completion date of the Beginning Educator Internship Exemptions: Student Support Staff ; Teachers with a teaching license prior to January 1992; Teachers with 5 years of teaching experience out-of-state. Part 5 -- Verification of at least 2 years of experience (both must be within the applicant’s endorsement area(s) and one must be completed in West Virginia) One school year of experience = 133 days. Form 4 Professional Certificate Permanent for Teachers and Student Support If you are applying for a permanent, you must complete the following on Form 4: Part 1 –Check PERMANENT (an applicant must either hold a Master’s degree or have held two conversion licenses) Part 4 – Provide the completion date Exemptions: Student Support Staff ; Teachers with a teaching license prior to January 1992; Teachers with 5 years of teaching experience out-ofstate. Part 5 -- Verification of at least 5 years of experience (two years must be within the applicant’s endorsement area(s) and one must be completed in West Virginia) One school year of experience = 133 days. Out-of-State Licenses If applicant opts to use a valid out-of-state license at the time of renewal, then applicant must use a Form 20A or 20S instead of Form 4. • Valid out-of-state license can only be used once. Form 8 Additional Endorsement Form 8 Additional Endorsement If you are applying for an additional endorsement, please complete the following on Form 8: Applicant’s Request College Recommendation--this must be completed by the issuing college/university and must have the original signature/date Additional Documentation **Submit the official seal-bearing transcripts reflecting your degree earned/awarded **Submit a copy of your Praxis II results (if applicable) Form 8 Additional Endorsement If the applicant holds a Professional Teaching Certificate endorsed in specified areas of special education, they can add an Autism endorsement with 6 college hours in Autism. No college signature is required and they do not have to take the appropriate Praxis II exam. Form 8 Additional Endorsement The endorsement grade level will match their current special education endorsement. Examples Multi Categorical 5 – AD THEN the Autism would also be 5 – AD Mental Impairments K – 12 THEN Autism would be K – 6 and 5-AD Form 8 Additional Endorsement If the applicant is using a valid out-of-state license, reciprocity will apply. No Praxis or official transcripts are required. Form 19 Initial, Renewal, & Permanent Professional Administrative Certificate Form 19 Initial Professional Administrative Certificate If applying for an Initial Administrative Certificate, please complete the following on Form 19: Part 1 – Check INITIAL Part 1a Part 2 – Must be completed by the issuing college/university and have the original signature/date Part 3 – Check “requesting an Initial” and verify 3 years of teaching and/or management experience Additional Information Submit the official seal-bearing transcript reflecting your degree earned/awarded Submit a copy of your Praxis II results in Educational Leadership Submit a copy of your Evaluator Leadership Institute (ELI) certificate *** If you are using an out-of-state transcript or license and have not completed your Praxis II and/or ELI training, you can be issued a Temporary Professional Administrative Certificate which is valid for one year only. This certificate cannot be renewed. Form 19 Renewal Professional Administrative Certificate If applying for a renewal of your Professional Administrative Certificate, please complete the following on Form 19: Part 1 –Check RENEW Part 1b – Select one option: 6 hours of college credit Master’s +30 60 years of age or older (provide a copy of your birth certificate) Part 3 --Check requesting the renewal Form 19 Administrative Technology Three of the 6 hours must be in administrative technology per Policy 5202. Policy reads, “must be a course related to increasing the capacity of public school administrators to improve instruction through the support of instructional technologies. ….should promote a vision for technology integration, assurances that administrators recognize curriculum, instruction, and environments that appropriate support the integration of technology, the use of technology to enhance professional practice and increase personal productivity, and the social, legal, and ethical issue related to technology integration” Form 19 Permanent Professional Administrative Certificate If you are applying for a permanent Professional Administrative Certificate, please complete the following on Form 19: Part 1 – Check PERMANENT Part 1b – Select one option: 6 hours of college credit Master’s +30 60 years of age or older (provide a copy of your birth certificate) Part 3 – Check “requesting a permanent” and verify 5 years of appropriate experience. Two years must be within a combination of the applicant’s administrative endorsements and one must be completed in West Virginia. Form 26 Content Endorsement for Special Education Form 26 Content Endorsement for Special Education Applicants must currently hold a Professional Teaching or Alternative Teaching Certificate in: Autism 5 – AD Emotional/Behavior Disorders 5 – AD Specific Learning Disabilities 5 – AD Mental Impairments Mild and/or Moderate 5 – AD Multi-Categorical (LD, BD, MI) 5 – AD Form 26 Content Endorsement for Special Education Applicants can use the appropriate Praxis exam to earn an endorsement in a subject area in place of coursework. **First time applicants for Form 26 can apply for any/all areas (English, Mathematics, Biology, General Science, or Social Studies) without any course work. The initial and subsequent renewals are issued for only one year. Continued… Form 26 Second and third year applicants must have 6 hours of coursework filled in on Form 26 and have official transcripts documenting the coursework. ( eLearning courses can be documented on an official transcript OR on an eLearning certificate) **At end of third year, if all coursework is completed in the endorsement area(s), the applicant will be issued a permanent endorsement. **At end of third year, if all coursework is NOT completed, the endorsement area(s) will expire and cannot be renewed. Advanced Salary All graduate level hours reflected on an official transcript of an appropriatelyaccredited college/university, related to the public school program or to the specific endorsement area(s) held, may be used for advanced salary on a professional certificate. Advanced Salary A maximum of 15 undergraduate level hours (including post-baccalaureate and other non-graduate level hours) may be used for salary IF completed after the effective date of the initial WV professional teaching, student support, or administrative certificate.
