☐ MATH 1342 (Elementary Statistics) – 3 hrs
☐ Abnormal Psychology
☐ BIOL 2401/2402 (A&PI / A&PII) – 8 hrs
☐ PSYC 2312 (Developmental Psych) OR PSYC 2314 (Lifespan Growth & Dev. Development of
personality and learning across the life-span) -3 hrs
☐ Medical Terminology -3 hrs

All prereqs must be completed by the time of application and you must receive a grade of at
least a C in every course.

Must maintain a GPA of at least a 3.0 in the last 60 hours of undergraduate or last 12 hours
of Graduate school. This is an advantage if you are unsatisfied with your GPA prior to the last
60 hours. They will only pay attention to your most recent grades!

You must take the GRE

Acquire a minimum of 20 shadowing/volunteer hours under an occupational therapist and
they must fill out the MOT Applicant Clinical Observation Form

“MOT Applicant Academic or Professional Reference Form to be completed by an individual
who can best attest to your readiness for graduate level work. Reference from a
college/university faculty member under whom the applicant has completed a course of
study is preferred; however a professional reference from an employer or supervisor will
receive equal consideration.”
