The course will include goal analysis, identification and specification of client objectives, case planning, client contracting, data collection, record keeping and client progress evaluation and reporting. Skills are applied using actual client examples. Participants will be expected to write case plans, client contracts, and treatment goals. You will be expected to demonstrate mastery of all other case management work skills. (UC, CSU, CAN Psy 2) Required Text: Psychology: Fundamentals of Case Management Practice, Nancy Summers. 3rd. Edition, Brooks/Cole Cengage Learning. General Aims: 1. To provide understanding in the process of effective case management. 2. Gain knowledge of efficient method of case management. 3. To become familiar and aware with referral sources in the community. 4. Analyze both objective and subjective methods of progress evaluation. 5. Understand the development of discharge planning. 6. Understand the development of progress writing and demonstrate your ability to write a progress note 7. To come to appreciate some of the workings of the mind. 8. To develop an awareness of the methods of inquiry used in the field. Obligations of the Student: 1. Read scheduled modules ahead of class and be knowledgeable and prepared to discuss chapters. 2. Attend each class as per the college attendance policy. 3. Participate in class discussions, exercises, and group activities. 4. Completed required tests, group projects and assignments on time. No make up or late work will be accepted. 5. Please make sure you put your name and the class time on your papers. 6. Give your full attention to the instructor or student who is speaking. You are responsible for all materials presented and assignments made during an absence. Individual Presentation (Due April 2nd) You will be expected to visit a community agency and inquire about the type of services they provide, who is eligible and the current target population they serve. You will also need to identify the type of case management the agency provides. You will write a 3-4 page report (double space with cover page) and you may add flyers or pamphlets from that agency. These do not count as pages required for the paper. You will present your report to the class (5-10 Min is sufficient) Report = 50pt + Presentation = 50pt. Total of 100 points Group Case Study/Activity (Due April 16th) You will have the opportunity to create your own case study using the psychological methods in case management you have learned in this class. You will work in groups of 3. Group activity will be the last 20 min. of class. Your group will present your case study and findings during your group case study presentation. The group case study presentation is worth 50 points per student. Exams: Type: There will be 2 exams and 1 final exam. Each exam is worth 100 points. Coverage: Exams will cover anywhere from (four- six) chapters from the text, lectures, and presentations. There is not enough lecture time to cover the chapters completely. You will, unless instructed otherwise, be responsible for the entire content of assigned chapters. Dates: Exams must be taken on designated dates and times. They may not be taken late. No make ups are available. If you have a school activity that will require you to be off campus or out of town you must arrange a test date with the instructor. Final Exam: The final exam will cover the last 7 chapters of the text book and lectures. The date for the final is Thursday, May 21st. Attendance: You must attend all classes. According to the Delta College policy, only 9 absences for MW are allowed. It is the student’s responsibility to keep track of absences. Grades: The total number of points earned throughout the semester determines final grades. Minimum requirements for grades are based on the following: 90% - 100% = A 89% - 80% = B 79% - 70% = C 69% - 60% = D 0% - 59% = F THERE ARE NO MAKE UP TESTS NOR ARE LATE PAPERS ACCEPTED REGARDLESS OF THE REASON.
