(Please note: York Region District School Board does not provide transportation for students attending the Arts
Unionville program. Students who are accepted into Arts Unionville will be charged a program fee of $100.00

APPLICATION INFORMATION (please read carefully):









Students may apply to the following disciplines:
DANCE
DRAMA
VISUAL ARTS
MUSIC (Vocal, Piano, or Instrumental)
Applications received at the Arts Unionville desk, in the main office, by Friday, December 2, 2011,
4:00 p.m. are guaranteed an audition.
Late applications will be held in order of the date and time received. An audition will be scheduled only
if time permits.
A maximum of two auditions is available to each student.
A separate application form is required for each discipline (maximum of two).
Students and their parent(s) and/or guardian MUST reside in York Region (no exceptions).
A separate (non-refundable) audition fee of $20.00 must accompany each application.
Please let us know about your child’s upcoming school trips so that your audition can be scheduled
accordingly.
Because of the large number of auditions being arranged, we may be unable to change assigned
times. Auditions will be held January 26 – February 1, 2012.
ATTACHMENTS REQUIRED :

Audition Fee: A separate cheque for $20.00 (made out to ARTS UNIONVILLE, with the student's name
written clearly on the front) must be attached to each application.
Student Questionnaire: Please attach a separate sheet with student answers.
Parent Questionnaire: Please attach a separate sheet with parent answers.
Academic Teacher Reference: Completed form must be sealed in an envelope by the reference.
Arts Teacher Reference: Completed form must be sealed in an envelope by the reference.
Report Card: Please attach a photocopy of your final June 2011 report card.
Proof of Residency: ONLY for students NOT currently attending a YRDSB school.
HOMEOWNERS - Please attach a photocopy of ONE of the following:
o Purchase agreement
o Municipal tax bill and property assessment
RENTERS - Please attach photocopies of the following:
o Lease Agreement and 2 of the following: home phone bill, hydro bill, credit card bill, cancelled
cheque for rent payment
Student Photograph: Please attach (where indicated) 2 copies of a recent “head and shoulders”
photograph. A school photo is acceptable. (Please use either a staple or paper clip to attach photos –
please do not use glue)
Dance Photograph: Those applying for DANCE must also include a "full body" photograph.
ARTS UNIONVILLE INFORMATION SESSIONS (you must call Markham Theatre to reserve seats):
Two Information Sessions will be held on Tuesday, November 1, 2011, AT THE MARKHAM THEATRE. To reserve
complimentary seating, please telephone The Markham Theatre Box Office at 905-305-7469 starting October 3,
between 11:00 am and 7:00 pm. Be sure to specify which session you wish to attend:
Session 1: 1:00 - 2:30 pm - A general information session plus Arts Unionville performances will be held at
Markham Theatre from 1:00 - 1:45 pm, followed by a short question and answer period. This session is primarily
for students accompanied by their teachers.
Session 2: 7:00 - 9:00 pm - A general information session plus Arts Unionville performances will be held at
Markham Theatre followed by specific information sessions in the Arts Studios at Unionville High School. This
session is primarily for students accompanied by parents/guardians.
